Office was paid $500 to complete a form for lawsuit. Has been paid over 90 days and has yet to provide form.I was in a car accident 12/20/14 in which I was the passenger of a rear end collision. I received care from Family Medicine and Rehab from this time until 6/2015, in which the doctor referred me to a neurologist and released me from his care. My counsel has been attempting to collect a Permanent Partial Disability (PPD) form to send to the insurance adjuster about my settlement. I authorized my counsel months ago to pay $500 that Family Medicine and Rehab charged to complete the form. On 4/1/16 I received a call from my counsel that they had not received the PPD form. I called Family Medicine and Rehab, and [redacted] answered the phone. I informed her why I was calling and she placed me on hold. I was in the neighborhood so I walked into the office as I continued to be on hold. I was told to come on back. [redacted] remembers seeing the form, but can't locate it. [redacted], **. [redacted] nurse, came out and informed me she had just hung up the phone with my fiance, who was in the driver when the accident occurred. [redacted] informed me that **. [redacted] would be completing the form when he returned to the office and my counsel would have it by 4/5/16. My counsel contacted me in July to inform they still have not received the form. I have called the office and left messages. I have [redacted]'s personal cell phone so I have called and text and no response. Family Medicine and Rehab has been paid months ago to complete a form documenting my personal injuries. I understand they have other patients, but I have been patient. I have not been seen by this doctor in over a year. I just want my documentation that has been paid for to move along with my life.
