was developed and used to provide treatment to these Three people which included individual sessions, family sessions, rehabilitation services, group sessions and case management. In addition, there was discussion for possible inpatient services for Ms. [redacted] at the time of her treatment which she declined and wanted to complete treatment in an outpatient setting. All charges billed to Sooner Care were for service provided to Three people in the [redacted] family as agreed upon by Ms. [redacted]

The signature on the forms are not mine. [redacted] was never seen by a counselor from Family Recovery at his school. [redacted] was only seen once in the home.

In response to Ms. [redacted] rebuttal, she sought this agency to provide treatment for her sons because of issues they were experiencing at home was spilling over into their behavior at school (see attached documents). Her son's treatment program centered around emotional behavior and how it affected their performance in school (see attached documents). We were working with Ms. [redacted] to get Individual Educational Plans (IEPs) for both of her sons. Treatment for this family was very inconsistent over the year time frame when treatment was provided due to lack of participation by Ms. [redacted] so there a breaks in services.
As far as "The signature on the forms are not mine", this agency has three files that have the same signature for all documents which are signed by Ms. [redacted].

Again they have billed and been paid for services that they did not provide. They can not just get IEPs for a school aged child, the professional staffing at school test them for IEPs has nothing to do with Family Recovery. Family Recovery are the ones that did not show up for appointments had the Family waiting, at home where the counseling was suppose to take place and did not. Family Recovery still billed knowingly services were not rendered. The professional practice of the employees of Family Recovery are being investigated by other agencies at this time, through them my signature has been deemed forged.
