Review: Lack of communication with Client about money handling and unreasonable expectations of the Client being able to provide food and personal items on $25.00 per week. I feel that their could have been discussion and an explanation as to how the Client was suppose to achieve a healthy life with no provisions for food. I (the Client ) called and told them I was unable to survive on $25.00 per week. With Sheboygan Family Services being a rep payee for me they did not call back when they said they would to discuss this matter. I am a diabetic and need to have proper nutrition in order to keep my health.Desired Settlement: An appropriate increase in the weekly allowance provided by the agency and proper handling of my monies paid to me by Social Security Disability. I would like to be includied in the decision making process not just turn my funds over to them to handle any way Sheboygan Family Services deems fit.

We are the representative payee for the above mentioned Individual. In late February the Claimant

notified us that she had an obligation to the courts that needed to be paid by 5/19/14.

At that time we redid the budget to meet her monthly obligation and let her know the limited amount of

funds available for personal needs would have to be 525IWeek and that we would try to get her on budget

with the utilities to improve her weekly cash. After getting her on a budget plan and reworking her budget

we were able to Increase her weekly personal needs amount to $551 week effective 4/14/14. After the

court obligation we will again revisit her personal needs budget in late June and make any adjustments

then.

As representative payee we have limited funds to work with. When a claimant notifies us of court

responsibilities and requests that we make the payments on their behalf It usually results In reduce

personal needs money. This was explained to the Individual and a budget was sent on March 1. 2014.
