Per complaint *** *** as charged for multiple times for tobacco use counseling and has not received billing information on amounts owed
Fayette County Memorial Hosptial follows the Department of Health and Human Services (CMS) guidance for Couseling to Prevent Tobacco Use allowing for
two attempts per year. According to CMS, each attempt may include a maximum of four intermediate or intensive sessionw, with a total benefit coverying up to sessions per Medicare beneficiary who uses tobacco. Intermediate sessions are to be three to minutes, intensive are over minutes per attempt. Beneficiaries who use tobacco regardless of whether they have signs or symptoms of tobacco-related disease, who are alert at the time of counseling and counseling is given by a qualified individual. Our Clinical Practice Manager has reviewed all counseling sessions to ensure services were provided (by examining provider documentation, G codes, ICD codes, time notation etc.) and follow the guidelines above. Counseling includes asking about tobacco use, assessing the willingness to attempt to quit and patient monitoring. Education materials are also provided.
*** *** received one session for the year and two sessions for the year and has received several statements showing charges and account balances. Our billing office manager and financial counselor have reviewed her charges. Electronic remittance history was also reviewed to ensure correct Medicare billing of services provided for each visit. I will ask our billing department to mail (first class) out a billing statement 9/10/which will show all of the outstanding balances. I will also mail out our Financial Policy and Payment Alternatives and ask our financial counselor to contact her concerning affordable payment methods
*** *** was discharged from DrJ***'s care due to issues concerning the physician-patient relationship which is in compliance with our patient discharge policies. Please see ***s initial hand written complaint, "What is your complaint " section, a portion of the issues which led to the dismissal are explained there. A dismissal letter was sent to her on 7/20/2015.

I [redacted] am still rebuting the charge for tobacco counseling, From Fay County Surgical & Assts per Dr. J[redacted]'s care. 1st Medicare dosen't even know about it I talked to them personally, I also called Dept. of Health.[redacted] is who I spoke with from Medicare, There was nothing listed bu office visit from FMSS. They ahd never heard of this. I also talked with. [redacted] they did not know about it. ###-###-####I called and spoke with Health Dept. ###-###-####. [redacted] & [redacted] Medicaid Hotline ###-###-####. To No advail here. Talked to [redacted].Yes I have gotten paper about smoking even thow I told them not to give them to me. Rita S[redacted], Dr. J[redacted]'s asst. said she had to. My Mother had me, I had my daughter she is 32 yrs. they always asked if you smoke. I was never charged prior..., Also the message I recived was for [redacted]. My name is... [redacted]... I know I owe a balence but will not pay it til the tabacco counseling charged are gone. Please call Medicare, Medicaid, etc Health Dept.Not 1 of these buinesses has heard of this.I also recieved 2 dismissal letters. One Reg mail & 1 certified. This was after I raised cain for reciveing papers of tabacco use counseling on July 16th office visit with Dr. J[redacted], I also have a witness that she never even ask me if I was smoking.Please investagate further.Thank you,
