I am in receipt of your letter dated April 6, regarding complaint ID ***. Currently, I am in the process of reviewing the complaint with the doctor, our billing company, and the coding resources, etc to confirm we correctly coded for his appointments.Once I have hear back from
everyone, I will provide you with an update. What I can tell you currently is the following:Patient saw Dr*** as a new patient on 4/13/16.X-rays were taken.Patient purchased PowerSteps which are an over-the-counter, not custom, orthotic for $45.The patient was casted for orthotics and paid $partial payment to order the orthotics.Self-Pay amount for orthotics is $350.00, which is for orthotics only.Typically, we check insurance coverage on orthotics and get a prior authorization which states that insurance has approved the Durable Medical Equipment, but insurance companies have a statement that says they are approved but payment is not a guarantee And each insurance company and each plan has different rates they will pay against a claim so we do not know ahead of time what the insurance company will pay.I am not sure why patient decided to go forward with self-pay vshaving us obtain a prior authorization through their insurance company We do not advocate one way or another On 4/27/orthotics were dispensed and patient paid remaining balance.Appointments on 5/23/and 5/25/were cancelled.The patient was seen on 6/1/for an orthotic recheck.On 7/12/patient’s father called to discuss the outstanding balance of $for the dates of service seen.Patient’s father stated that he should not have to pay for office visits scheduled for follwith orthotics He stated he would pay the balance and instructed staff to cancel all future appointments which she cancelled the appointment for 8/10/16.No further payments were made after this phone call.Feet First Podiatry, Inc.’s billing process is the patient/guarantor receives statements and then a collection letter stating that if their account is not paid that they will be turned over to a collection agency.We turn accounts over to the collection agency at various times during the year.I am not sure where the cost of $came from because we charge $for self pay for orthotics, not including x-rays, office visits, etc.And, we do not have any documentation that we would run the orthotics through the insurance company.The medical assistant did document the phone call from 7-12-16.Because the patient already received bills and collection letter, no further communication from our company is made.Patient/guarantor made the following payments against his dates of service: 4-13-co-pay $25 4-13-deposit for orthotics $150 4-13-$for PowerSteps 4-27-$200 4-27-$101.76 Total $paidPatient’s current balance is $125.38; $outstanding for 4-27-balance plus the collection agency fee of 20% of $20.90.Patient’s father signed an OFFICE & FINANCIAL POLICY form that states the “Delinquent Accounts are subject to collection proceedings … all collection, attorney, and court fees shall become the patient / guarantor’s responsibility in addition to the balance due to the office.” See attached.We have never, to the best of my knowledge, been turned over to the Revdex.com We are hoping after confirming that everything we did was correct with coding principles, that we can come to an amicable resolution between both parties.We have decided to improve our process when a patient wants us to not prior authorize an item, procedure, etcand have the patient sign a form stating as such that they did not want to have a prior authorization done and they wanted to pay the self-pay rate. I received the Revdex.com original complaint and additional comments against Feet First Podiatry and the way[redacted]'s orthotics were handled and the rudeness provided by our staff. I want to apologize for anyrudeness. Our staff should treat everyone with respect, just as each patient, guarantor should treat eachstaff member with respect.I am not sure why the staff offered you a self-pay rate vs. obtaining a prior authorization from yourinsurance company. This is not a practice of Feet First Podiatry. We offer the self pay amount to thosewho don't have insurance or for those patients that specifically ask us not to run it through their insurance.However, we would not have said that you owed $500 for orthotics and that you wouldn't owe anythingelse for additional office visits, orthotic rechecks, etc. Somehow, there is a misunderstanding about thisbecause your son had an office visit, x-rays and bought Powersteps, all of which are not included in thecost of orthotics.Regarding your second issue regarding not receiving the bills, son turning 18 - collection agency,etc.:Attached is a copy of the bills that were mailed to your address from our billing company - 2 statementsand a collection letter. After no payment from two statements and the collection letter we do not send anyother statements. We do not have it documented that you told the staff to run it through insurance duringa phone call. I am sorry if the staff did not document that nor follow up with that.I am the one that turns accounts over to collections and I did not, and would not, look at anyone's age toturn a patient over to collections. We do not run a deceptive business. We run an honest business andwe try to do everything correctly within the realm of insurance companies, coding practices, etc.Regarding your third issue about the balance:We are writing off the current balance and I have notified our billing company and collection agency thatwe have written off the amount. Further, I have not instructed the collection agency to turn your son overto the credit bureaus, so the account going to the collection agency in no way impacts his credit.I am not sure if you contacted your insurance company or not, but they are the ones who determine thecost they will pay the medical provider. The outstanding amounts from visits 4/27/16 and 6/1/16 for$52.24 were both applied to your deductible, both of which were correct and allowable charges againstthe office visits. There are some procedures that have a global period, where if the patient is seen withina certain amount of time after the procedure that is defined by the insurance company, such as 1, 7, 10,or 90 days, certain fees are covered in the post-operative or global period and an office visit is notcharged. However, if additional services were done, i.e. an x-ray was taken, or another concern / issuewas discussed that was different than the first issue/concern, then those services and /or office visit is tobe charged per your insurance company.Again, I am sorry for your concerns and hope that the write off of the $125.38 will help.Sincerely,Julie D. We are rejecting this response.  First of all, it clearly makes NO sense why we would have not proceeded to use our "GOOD" insurance.  It was very clearly stated to us that if we pay the $500, that any further or future dates relating to these inserts, there would be NO charge.  Staff clearly stated that if we ran it through our insurance, First Feed Podiatry would charge more and was told we would owe more if they ran it through our "good" insurance.  Secondly, we NEVER received a second notice of a bill.  After the first bill, my husband spoke to "RUDE" staff about the balance, and staff was told to run it through our insurance.  Never heard another word from the company, nor ever received another bill.  We feel that First Feet Podiatry was waiting for my son to turn 18, then turn him into a collection agency, which we feel is very deceptive practices.  He was 17 at the time of service (with good insurance).  Father signed paper, but collection letter is in son's name......who was 17 at the time and never signed anything.  After speaking to First Feet Podiatry's billing department after the first letter, they had no answer for why I was billed any further amount, either, after paying the cash amount requested.Thirdly, we feel my son owes nothing.  The company hasn't suggested a way to resolve this other than turning him into a collection agency.  Just want to reiterate.....we have good insurance.  This clearly makes no sense.  Why not use our insurance, but for staff persuading/telling me to pay cash out of pocket.
