Thank you for your response,
I will make sure to go directly to the center to retrieve my test information to ensure not to confuse or inconvenience that staff, as an answer back still has not been gotten.

I originally went to FCI in November 2011 to have testing done. We were told fertility treatment was required due to inability to conceive naturally. We went back in June 2012 to start. In the meantime, my insurance had changed because of marriage and I provided that information. April 2013, after the birth of my son February 1, I received a letter stating there had been an audit on my account and I owe $2492.00. I had to call to ask why this was. They told me instead of switching my insurances, like they had said they were doing, they added the insurance for a secondary insurance. Because there wasn't a secondary insurance, the bills were not paid. I was given 2 months to pay the money or it was going to collections. I tried to set up a payment plan however it was due within 12 months and that was not feasible. We decided in 2012 to have the embryos and sperm frozen. There is an annual fee we paid. We were then due to have the embryos and sperm refrozen on July 1, 2013. I called early June to pay for these as we had the money. I was told the money would come out that day. A month later, I realized they hadn't ever taken the money out. I called back and was told that the price had gone up and I had to pay $150 more. I did this and thought everything was taken care of. July 6, 2013 I received a bill for $300 for the freezing of the sperm. This had already been paid for. I called the office to ask about it and I was told they didn't have anything in storage! My account manager was not any help so I spoke with a supervisor who ended up finding it but was unable to find any notes in my account of the previous exchanges. I was then told we would need to repay the $300 and the $2492.00 owed or the vials of sperm would be thrown out.
