First Choice Community Health Centers

Requested copy of dental records the 1st of September 2015. Several visits,and phone calls, but still no records.Promises made to me and not kept. The 1st of September I requested a copy of my dental records from the Anderson Creek Clinic. the request was faxed to [redacted] No response in 2 weeks, so I returned for follow-up. again request sent and phoned. No response. I went to Lillington office in mid-October. Appointment refused, [redacted] states allow 2 weeks. No response. Again I go to Lillington office, this time [redacted] says 2 days as she has my information. No response. I again phone [redacted] requesting dental records . no response. I send complaint addressed to Patient Concerns Representative Nov 10, 2015 by certified return receipt. [redacted] phones me twice on November 18, 2015. She states she is leaving and going to another corporation and the issue is given to someone else. She states once issue given to someone else she has nothing to do with it. she phones back and states she will call again in the afternoon with the contact information. But she never does. On Nov 25, 2015 I again go to Anderson Creek office for assistance. This time I am told the information was faxed to someone else concerning my situation. On November 30, 2015 I phoned the Lillington office and spoke with [redacted] He states he seen my name earlier. I make my request. He stated I may have to re-submit. I explain the original is at the Anderson Creek office. He stated he may have them send him the original. On Dec. 1, 2015 I contacted the NC Board of Denatl Examiners. I send a certified letter with the above information to NC State Board of Dental Examiners that was delivered on Dec 7, 2015.On Dec 9, 2015 I receive a call from [redacted] stating he would go to the Anderson Creek office next week. He phoned me Dec 17 stating he had my records, and we met Dec 18 but he give a copy of my application and none of the dental records. Again he stated we would go to Anderson Creek next week. That is the last I have heardDesired SettlementI would like to have MY DENTAL RECORDS
