The patient was in our office on 8/15/for a comprehensive oral evaluation, full mouth xrays, and full mouth debridementI have enclosed the clinical notes, the explanation of benefits from delta dental and the patient ledger to support the fees, delta discounts and what was completedThe
patient was in our office on 8/23/for a one and one half hour scheduled appointment and we were running mins behind schedule and I let patient know that we still would have her done in the amount of time we had her scheduled and she said that she couldn't stay she had somewhere else to be and we reschedule her to 8/28/Patient was in our office on 8/28/for upper and lower right side scaling and root planningIt was completed that day by *** *** the temporary hygienist with our office that day. *** *** does not allow for the full mouth scaling and root planing to be done on the same day. All Dental patients have to get their scaling and root planing done on separate visits I have enclosed the clinical notes, the explanation of benefits from delta dental and the patient ledger to support the fees, delta discounts and what was completedPatient was in our office on 8/29/for the upper and lower left side scaling and root planingIt was completed by *** *** the hygienistI have enclosed the clinical notes, the explanation of benefits from delta dental and the patient ledger to support the fees, delta discounts and what was completedAt this appointment patient was instructed that she will need to schedule a week re-evaluation appointment with prophy to make sure she was healing properly after the scaling and root planingThe patient scheduled for 10/9/before she leftShe did tell us that she will be losing her insurance after 10/1/and we let her know that the cost was $Patient came in on 10/9/and had the week re-evaluation and prophy done by *** *** the hygienist that was new to our officeWhen patient came up to leave and asked to pay her balance of the $194 she said that she didn't remember that she was going to need to pay and she wanted to know what the remaining balance over the $was Patient was told by *** that she would look it over and call her*** called patient on 10/10/and let her know that the $was the difference on the debridement appointmentThis is supported by the clinical notes attachedThe patient called on 10/17/and asked about her balance because she was confused let her know that the balance of $was from the 10/9/which was $and the other $was the difference of the 8/15/appointmentShe said that *** told her that her balance was only $I let her know that her balance was $and explained again what it was fromI let her know that she should have gotten an EOB from *** for that 8/15/appointmentshe said that she didn't have oneI suggested that she call and get oneShe said that she wouldThe clinical notes for this date are attachedI received a letter from the patient about the balance again and what she thoughtI reviewed it and sent a letter back to her explaining step by step what all the charges were, what the insurance paid, what the write off discounts were and any payments she made on the account so that she could have clarification. I also included the EOB's from the insurance for all dates of service in questions I have enclosed what I sent to her.If you have any other questions, please feel free to contact me*** ***

I spoke with [redacted] the business manager of our office and he personally spoke to patient and sent what information she was requesting approximately 2 weeks ago.
