all customer service representatives that I have been in contact with have been from the business the complaint is against. 
the credit card information (number and CV) was typed out in an email to Melodie at Hawaii Pacific Health and she typed the wrong information to be processed (the credit company confirmed this and I am trying to get written confirmation of this). On a separate date, a customer service representative told me over the phone that I could fill out an online form to make payment, however the form wouldn't accept the zip code (not a US zip nor is it my address). 
The payment would have already been processed with out the credit card address had the CV code been typed correctly. 
 
However, I received a letter in the mail stating that the payment was declined (because of the incorrect CV) in the wrong amount from what I owed. Meaning, had the correct CV code been typed, they would have been charging me $715 more than what I owed. In that sense, thankfully Melodie sent the incorrect information to be processed.
