As I stated prior, all of my office policy are presented to patients in writing As I previously stated, I am happy to give you copies of all the policies dealing with this issue I am more than happy in this case to waive the $missed appointment fee

We are sorry for the confusion with your dental care. HIPPA does not allow us to directly respond to your issue on a public forum. What we can do is explain our office policy in an attempt to clear things up. When our staff makes appointments we explain what is to be done and give
an ESTIMATE of the patients portion for the procedure. We have no way to determine the exact amount that insurance will cover. They do not provide this information to us. When we call to confirm appointments we reiterate what is to be done and give estimate again. On date of appointment doctors assistants and/or hygienists reiterate with patient the procedure once again. This way patient has been made aware up to three times what is proposed. We try to honor all patient requests for treatment and materials. We strive to rectify any patient dissatisfaction immediately. As long as we are made aware of situation in a timely manner. We prefer to perform the best possible care and use the best materials. Unfortunately, insurance companies prefer to reimburse for the cheapest and most times least optimal procedures. We file all insurance claims as a courtesy to our patients. But we cannot know all the intricacies of all the available dental insurance plans. If you would like specific details made available on a public forum we would need a signed HIPPA release form allowing us to do this. Any issues you would like me to address need to come directly from the patient and not a spouse or family member

Dr Claiborne did not address any of the issues directlyWe have cell phone records showing we called his office a week prior to the appointment before we cancelled itShane Claiborne DDS office stilled charged us a now show feeWe should not have to pay that and before the bill had a $creditWe would like to be refunded that amount instead of receiving a bill from them for a no showAs mentioned in the my previous email, the dentist office never sent my records to my new dentistThey did not send my x-rays even AFTER I filed out the paperwork for them to be releasedThe result of that was me having to pay $out of pocket for new x-raysI would like to be reimbursed for that as wellThose are the issues two situations I would like addressed by DrShane Claiborne's office.

I do not understand why the office would not be able to respond to situations that I have already made publicThis has nothing to do with HIPPAI am glad that the office has these extremely thorough policies, however, they did not follow any of them in our caseWe responded the moment we received the bill (stating my insurance did not cover the material used)The office took weeks to respond to us via phone and gave us very little informationThe office staff and dentist constantly gave us the run around and would not work with us at all in this matterThey were rude and disrespectful when I and my father came into the office to speak with the office manager on this matterTo the point where they would not even allow us to talkThe dentist accused my wife of being a liar and cut me off every time I would try to speakMy father and I remain calmed, while the dentist became obviously angered and asked us to leaveWe paid the bill at this time due to the threats they made to send to collectionsThroughout the past few years we have paid over $3,in dental bills to this officeWe have always paid on time and trusted them to hold up their end on estimatesI am not pleased with their generic response to this issue

As I stated prior, all of my office policy are presented to patients in writing.  As I previously stated, I am happy to give you copies of all the policies dealing with this issue.  I am more than happy in this case to waive the $25 missed appointment fee.

I received your letter, case number [redacted], regarding one of my previous patients.  In order to remain HIPAA compliant we cannot release information regarding patient visits, treatment, proposed treatment or chart notes.  I can however provide you with some information in general...

regarding my office policies.   We instruct anyone requesting records to fill out the appropriate record release form.  We do provide records for our patients, but as is standard within any medical/dental office, we allow for 5 business days to copy and send records.  We do not charge for this service as some offices do.  But by law, we require the request in writing and provide a form to the patients upon request.  It is not standard for a patient to come in and demand to be handed records on the spot.  The form states "Please allow one week for processing".   But, once the release form has been filled out our office releases records on Fridays. We ask all our patients to be respectful and courteous and give our office 48 hour notice if cancelling or rescheduling.  All our patients sign a financial agreement upon their first visit, that does specify any cancellation with less than 48 hours notice are subject to a $50 cancellation fee.   This allows us to properly plan our day and schedule patients effectively.  Our office provides patients with forms outlining our financial agreement and office policies which they sign prior to being seen.  We can provide you with blank copies of all our forms if you wish.  We encourage all patients with questions or concerns to contact our office manager [redacted] or me directly.  My cell phone is available on the answering machine and the website email goes directly to me. Respectfully,  Shane R.
