In July of 2018 I had a sleep study ordered by Dr. S of ENT. I did not know that a doctor at the Ft Wayne Neurological center had read this exam. I never was seen by a physician at Ft Wayne Neurological Center. Therefore, I was not aware that I should even be looking for a bill to arrive with charges. My insurance was never billed. This is because they used an address and insurance information they had on file from approximately 2003. They billed that insurance policy and it was denied because it was no longer current. They sent my bill to *** in Fort Wayne where I have not resided in 8 years. I never received a phone call because in the past 16 years my phone number has changed.
I have contacted both FNC, and ENT to see where the break down in communication has been. Apparently ENT never received a phone call to verify my address. Instead, after months of nonpayment it was turned over to *** Collection Agency. Dr. S's office (ENT) informed me that it is not unusual for the FNC to contact them if it appears they have incorrect billing or insurance information however, on my behalf they failed to do so. Dear sir or madam, I pay my bills and I pride myself on the fact that I have excellent credit by being a responsible citizen. Which brings me to my biggest complaint.
Just a few weeks ago my wife and I applied for preapproval for mortgage as we were considering buying a house. It was then that I learned that my credit score had dropped from over 800 less than two years ago to 630! Upon investigation, we learned I owed a collection agency on behalf of the Ft Wayne Neurological Center. Having not been to that facility in at least a decade, I couldn't understand why I even owed them a dime. It took hours of phone calls to get to the root of the problem. I paid *** Collection Agency the money owed to restore my credit. Although I am not back to where I once was, it did go back up. However, I can no longer file this with my insurance to recoup some of the money that would have been covered by my health insurance policy because it has been too long.
I feel that the actions of the Ft. Wayne Neurological Center billing department were unethical and careless. It negatively impacted my credit reputation. Because I was not afforded the opportunity for this to be billed through my health care insurance policy to whom premiums are paid each month, on time, I had to pay more for services I did not realize were rendered. This issue was handled inappropriately. Once they either had nonpayment or my bill returned to sender, once they called the incorrect number they had listed or once the insurance they had on file denied my claim you would think that someone would investigate as to why EVERYTHING they had on file in regard to me was incorrect one would think you would realize the information was not current. I was never in this office. Again, I did not know a doctor there would interpret my sleep study. I never had the opportunity to provide correct information and they never took the initiative to understand why. People move, change insurance, change phone numbers in a span of 16 years.
It just isn't right what they did. I have spent hours and days trying to correct the harm they did they did an no fault of my own.

Actually this patient is not in our system and never came here for a study. I looked in FWN's WebChart and apparently Dr. H read a home study for Dr. S one of the ENT docs. They do their own home studies and the FWN docs read the study. Dr. S’s office passed the patients address and other demographics to our billing company for billing. We apologize for any misunderstanding.

I am rejecting this response because: Upon the discovery of this issue we spoke with ENT. At that time they explained to us their process. The sleep study was done and sent to FWN to be read by the neurologist. It is true, I was never seen in the office. I was completely unaware that anyone associated with FWN would be in anyway attached to my health care pathway. The response by Donald R. S, MBA claims that Dr Ss office sent them my patient information with my sleep study. I believe this to be false. Prior to 2018 I had never been seen by ENT. On my first visit I reported my correct, current address. They would have NEVER known I ever lived on *** Ave. Many years ago I had had a different sleep study (approximately 2004) at FWN. When I was there all those years ago I reported my correct address at THAT time which was ***. Again..... Dr S's office would have had no way of ever knowing I once resided at *** Avenue. It is my belief that the FWN billing department pulled my old address from THEIR records for whatever reason. In no way was this my fault. Dr S's office is not to blame for the bill being sent to *** Ave. The failure resides within your own billing department and ultimately I paid the price for something that could have and should have been handled differently. This issue is not resolved.
I have CC Kelly G from Patient Accounts with ENT. I am also attaching an email that she sent to FWN in which, to my knowledge, she never received a response.
FWN has damaged my credit reputation. I work hard and pay my bills. I have earned an excellent credit - it was not just handed to me. With their carelessness, it has impacted my credit score and cost me money that should have been covered by my insurance which I also pay for. They seem to not be bothered by how their carelessness has negatively impacted my life. It is INCREDIBLY wrong that I should now have to prove myself in the credit world because I was falsely and carelessly sent to claims because of their failure.

I received correspondence from the client on 9-9-19 about this acct and I requested it to be removed from credit at that time. The consumer was paying the client and they would report it to us.
Consumer then paid our agency in full on 9-10-19, which posted here 9-11-19.
Consumer was informed of update to credit on 9-10-19 at 11:29am during a call with an acct rep.
