I went to the office to see [redacted] he told me my left chest swelling was from a broken rib yet I brought him an xray that shows no break He then sent me to do more xrays I begged him for catscan and mri referals he said it wasnt necessary I went back to his office three days later to see [redacted] he saw my chest and immediatly sent me for mri and catscan and said its a possible artery clogged near my heart....I have been called three days after that appointment after endless callin for my referal annette from referal department said she was working on the referals I never got a call back and two days after that I call her and she went on vacation for next daysMeanwhile no call or care about anything now because they lagged my insurance dropped me because I failed to do a verification form that they never sent me and I have to wait days to have insurance If annette would have told me insurance insnt covering I would have been made aware early enough to complete the

They did not give me a *** nor talk about or do anything about my headachesThey said, once I paid my copay and after I was sitting in the office that this visit is a meet and greet and I would not have a *** done and would need to come back for this to be doneI took a day off work and spent $to talk to the doctor who asked me the same exact questions that were on the forms I completed
Possibly one of the worst experiences I've ever had at a doctor's officeThey don't return calls within the promised 72hrs, they care more about money then they do patient care, they have attitudes and in order to get anything done I've had to show up at the office many times and demand helpThis placed raised my blood pressure...don't go
They did not resubmit either claim to ***, my health insurance companyThey left off the correct taxonomy claim and both claims were denied
I changed insurance on 1-1-and was given their name from *** as someone in my networkI first visited them on January 23, I had an extreme back ache and hoped they could help me with itI also needed a referral to my Oncologist They told me that I must have a physical first and then they could prescribe medicine for my backI agreed to the physical They referred me to *** *** for an MRI on my backIt was a couple of weeks before *** had an openingI waited the couple of weeks and on the morning of my appointment *** *** called me to tell me that they did not accept my insuranceI called Forte Family Practice back and they then referred me to *** ***Another couple of weeks passed before they had an opening I arrived at *** *** to find out that Forte Family Practice did not s
DrForte failed to submit necessary paperwork to insurance company for them to pay bill
On November 14, DrForte requested a blood draw on my husband *** She neglected to explain to my insurance company the details of the blood draw and they will not pay the claim All her office needs to do is send in an explanation as to why she requested one of these tests and they will pay the bill Unfortunately after requested and continual conversations with Christina in her office no one has bothered to still to this date send *** this information and the bill is in collections now
