As I stated previously, my wife did agree to pay for the paperwork, however when she got home and reviewed the paperwork, the information was incorrect, that is why we continued to call and try to speak with the Stephen Kuder, to see if he could correct the paperwork, but he would not return the phone callsAlso, there might be now, but at the time that this all took place there were no signs posted of a $fee for paperwork I have no problem paying for paperwork being completed, but the paperwork was incorrect, their office people were told that and I do not feel that I should have to pay for something I did not receive

In this second response and in good faith, our position remains,a $charge for all formsthat require our Provider's time to fill out and their signature (this standard fee stands in most medical offices)Upon piof this form by M:rsNelson she agreed to pay the $charge and she was handed her first invoice.Due to Rulon Nelson, rude and ,abusive behavior to our staff he is no lon considered a patient here at Fortuna Medical Group Ialso hope that th Revdex.comcan see that the money is owed andshould NOT be posted as an unresol ed issue because a patient is non-compliant and does not want to follow po ted rules

                  [redacted]This is a very simple response:A medical form was brought in by the Patient’s wife and medicalinformation needed to be filled out.  All medical forms that needto be filled out by our Provider have a $25 charge by Fortuna MedicalGroup.  This charge is standard practice by medical clinics.  When the Patient's wife picked up the form it was explained that a $25 charge is required (this is alsoposted at the front desk for all patients to see and be aware).   The Patient’s wifeexplained they would pay.  A 2nd Request has been sent. Respectfully submitted, Fortuna Medical Group
