third-party claims processing, formulary administration, benefit plan
communications, and other similar activitiesExpress Scripts’ home delivery

Ms*** expressed concern with the inability to receive two
medications, overnight next day delivery, from Freedom Fertility resulting in
her having to secure both medications from an out-of-network retail pharmacyMs*** states that she received approval

Upon review of Ms***’s account, Freedom Fertility was
unable to dispense both of the medications in question on the same day as
requestedTherefore, Ms*** was
advised that both medications could be filled locally in order to have them in
time to complete her therapy cycle
Ms*** was able to locate a pharmacy to provide both
medications; however, it was an out-of- network pharmacy. In an effort to assist Ms***, Freedom
Fertility offered to use a credit card to pay for the medications at retail but
the retail pharmacy would not accept Freedom Fertility’s credit card. Ms*** was advised to submit her receipts
for reimbursement for medication
The retail pharmacy processed a different prescription than the
mediation Freedom Fertility had on file.
The physician called the prescriptions in directly to the retail
pharmacy which typically must go through Freedom Fertility first who would then
refer over to the retail pharmacy. As a
result of Freedom Fertility not having the prescription, reimbursement was
denied. Ms*** must submit for
reimbursement directly to the prescription plan sponsor who is fully aware of
the circumstanceThe plan sponsor has
paperwork that Ms*** must complete for direct reimbursement which is located
on her plan sponsor’s websiteThe other
medication in question will reimbursed, less her co-payment, in accordance with
the prescription plan guidelinesMs

These services, contracted by the plan sponsors, satisfy the plan sponsors’ goals of providing a cost-effective, seamless and clinically appropriate benefit for its members.Ms. [redacted] expressed concern with the processing of her medication in May 2016 from Freedom Fertility.  Ms. [redacted] indicated she paid for the medication without using her insurance and obtained with intentions to file with her insurance at a later time.  Ms. [redacted]’s claim was denied by her medical insurance since they indicated that the purchase should have been processed at the date of sale using her insurance plan; therefore, she is requesting reimbursement of $5,700.00 from Freedom Fertility.November 8, 2016Page 2Ms. [redacted]’s order was originally received in March 2016; however, she requested the order be postponed. Freedom Fertility maintained the order on file as requested.  Upon receipt of Ms. [redacted] call to Freedom Fertility in May, the order was placed for the medications in question. The calls in question were reviewed and at no time did Ms. [redacted] indicate she had insurance coverage; therefore, would be paying out-of-pocket for her medications. Upon receipt of the order, delivered on May 5, 2016, Ms. [redacted] called the following day stating that she received medications which she did not order. In listening to the call with customer service, the representative provided a list of the medications at the time of the order and proceeded to process them as requested.  Therefore, Ms. [redacted]’s request to return the medication was denied. On August 26, 2016, Ms. [redacted] contacted Freedom Fertility advising that she was trying to submit for reimbursement to her insurance company; however, the request was denied.  As a result, Ms. [redacted] inquired if Freedom Fertility would be able to bill her insurance and provide the credit.  This request could not be completed since Ms. [redacted] needed to provide the insurance information at the time of the order.  The only option for Ms. [redacted] was to submit her receipts for reimbursement to the insurance company directly. Ms. [redacted] contacted her insurance company in October 2016 to assist with filing the manual claim; however, Freedom Fertility is unable to bill the insurance for the past date of service. These services, contracted by the plan sponsors, satisfy the plan sponsors’ goals of providing a cost-effective, seamless and clinically appropriate benefit for its members.Ms. [redacted] expressed dissatisfaction with her experience with Freedom Fertility resulting in her having to purchase the medication at the retail pharmacy.  Receipts were received for the retail purchase and reimbursement was processed.  In addition, Ms. [redacted] express concern with the six month expiration date for medication received from Freedom Fertility.  The medication in question is a compound drug; therefore, has a shelf life of six months.  Revdex.com Serving Eastern Missouri and Southern IllinoisJuly 20, 2016Page 2 In Ms. [redacted] complaint, she also stated she was advised her medication were not covered.  Based on Ms. [redacted]’s prescription plan guidelines, the medications are covered with the exception of one compound.  Ms. [redacted] was informed of this information and approved Freedom Fertility to process the non-covered compound which she would pay out-of-pocket.   Ms. [redacted] indicated that there were items missing from one of her shipments; therefore, she contacted Freedom Fertility and sated she was informed that the medication would be mailed yet she was charged again.  There is no indication Ms. [redacted] was charged for medications without her permission.  The last three orders set up in May/June were processed after the patient was called and payment provided.
