I received two gift certificates for a 1-1/massage, each from this establishment Upon my first visit, I used one certificate for a 1-1/hour massage I was told at the end I was all good and everything was taken care of When I arrived for my second massage I was informed I owed $for the previous massage (they had no record of me using the gift card) and the certificates I had were only worth 1/hour massage, not the 1-1/as printed on the certificate I used the second certificate to pay down what I owed from the first appointment and cancelled my second appointment They said they would bill me for the outstanding balance on the first appointment The gift certificates have been printed to be misleading to the user, and I felt taken advantage of in the settlement of past services

February 27, To Whom it may concern: This response is in regards to a former patient of Freedom Physical Therapy Services Patient Michael *** case #***While Mr*** complained about services at Freedom Physical Therapy, his issues were really with the policies of
his insurance provider. His insurance provider had rules with respect to using multiple offices for services. There was paperwork required in order for the insurance company to pay the health care provider. Unfortunately, the paperwork that would have allowed Mr***’s visits with Freedom to be paid by insurance was not completed. Mr*** had been seen by Freedom on two occasions prior to Freedom becoming aware that he was being seen at multiple offices. Therefore, his insurance company denied payment to Freedom for our services While Freedom completed the necessary paperwork and sent it to the insurance company, Mr***’s previous provider did not complete their part of the paperwork process. Again, due to requirements of his insurance provider, the two visits with Freedom were denied by his insurance company Mr*** has received three statements to make him aware of balance for services rendered and he did call on two different occasions confirming that he received statements is aware of balance and is currently working with other facility to send their paperwork Mr*** stated he called the other facility and he was also going to call his insurance provider to confirm information was received from other facility so denied claims can get sent back for reconsiderationAccording to his insurance provider the other facility never received a call from Mr*** and as a result paperwork was never sent to the insurance company Since the paperwork had not been completed or follow up to the previous facility, I contacted the insurance provider who was extremely helpful in getting the paperwork needed in order to process the claims at Freedom for Mr*** So in the end, this issue was resolved for Mr*** through the additional efforts of Freedom and his insurance provider This is an important example of how all consumers need to be aware of their insurance coverage, procedures and make sure they follow up on any charges based on the information they receive from their insurance provider.
