Fresno Medical Center [redacted] , MD Internal Medicine [redacted] CA Phone (###-###-#### Fax ###-###-#### [redacted] Website: www.fresnomedicalcenter.org December 26, Business BureauW Shaw Lane Fresno, CA Dear [redacted] , RE: ID The complainant has been a patient in our office since 11/01/when he was seen by our physician for a New Patient Office visitDuring this visit, the patient requested that a work physical form be filled out and completed, which was done by the Doctor and Medical Assistantlaboratory tests were also done on this datePatient returned on 11/15/for additional further testing and another required laboratory test was done to confirm his medical diagnosis, Our office policy in regards to insurance coverage and patient financial responsibility is that all insurance plan deductibles, copays and co-insurance amounts are due and payable at the time of serviceThe patient's coverage is after their deductible with a co-insurance amount of 20% after the deductible has been met, for all chargesThe patient returned for a followup visit and for his test results on 11/29/2017, After informing the patient of their current balance due the patient asked to speak to the billing department, to explain their billing statement to themDuring this discussion, the patient's personal insurance coverage and eligibility were also both explained in detail to the patientThe patient then stated that they just came in for a free well visit and to have a work physical done and farm filled outIt was then explained to the patient, the difference between a work physical and a preventative well visit physical, the latter of which we only do after a patient has become established and on their birth dateA well visit physical is a yearly physical for preventive care and not a work physicalThe patient became persistent that their insurance pays 100% for work physicals and he only wanted his "free visit"He was demanding that we recede our charges and rebill it to his insuranceAfter this lengthy discussion, the patient was giver: a discount off his total balance and paid his current balance owing&he then cancelled his appointment scheduled on 11/29/and left the officeHis issues were resolved according to his wishes [redacted] MD
