This is in response to claim ID ***.The patient came in initially for an eye examination on May 29, The patient later contacted our office since he was interested in trying multi-focal contact lenses and, therefore, was scheduled on January 7, for a multi-focal contact lens
fitting which involves the initial evaluation and measurement of the corneaSince the lenses were of a specialty type lens, trial lenses were ordered and the patient returned on January 14, for the assessment of the lenses on the eye checking for any adjustments on the prescription and lens fit, as well as the instruction for insertion and removal of the contact lenses since it was the patient's first time with contact lenses.
To avoid using the VSP benefits, patient paid $out of pocket for the contact lens fitting.
There was a miscommunication with the billing department and the insurance was billed in error.
As per the patient's desired settlement, all of the patient's VSP benefits have been reinstated so the patient can now use his benefits for glassesThank you so much for consideration of this matterIf we can be of any further assistance, please do not hesitate to call
