riddled with misspelled words and very insulting to the claim adjusterHis
chief complaint is that we have his name as Jon instead of JohnHis workers
compensation injury from was submitted to the Gallagher Bassett

employer with his name as Jon and not JohnWe are responsible to report this
to the Virginia Worker’s compensation commission the same way we receive itIt
was the claimant’s responsibility to notify the VWC of the name correctionWe never
received any notification from the commission to correct his nameWe never

did not contact the adjuster and left her a message with the proper spelling of
his nameShe returned his call and left him a message notify him that it was
reported to us this way and that he needed to notify the commission

Review: I keep on gettting mail addressed to [redacted]

For one that is not my name. In 2014 I did not work for [redacted].

I keep on getting letter from this [redacted] Phone number [redacted] about working for [redacted] and now [redacted]. These letter are addressed to [redacted] For one I do not use this first name. My name is [redacted].

Two, in 2014 I worked for STS Line Maintenance. Their address is [redacted]. I worked for the customer " [redacted] Address [redacted].

I have never worked for [redacted]s nor [redacted]. Nor do I go by the name as [redacted] as she is claiming.. I also can not sign my name as Jon. I am an [redacted] What she is telling me to do is 100% fraud. She and her company were notified time and time again all of their mistakes. But she and her company has been refusing to correct her spelling and miss information. Perhaps she has never passed the first grade.

The line Maintenance Manager, [redacted] Phone number is [redacted]. If you need to contact him about this matter. He is located at the [redacted] in Washington, DC.

Now as far as my figure is considered. No feather action is necessary. It is healed and repaired. Their are no feather medical bills. That is if you are referring to [redacted]. This [redacted] I have never heard of him or she. This is not my name.Desired Settlement: Send Ms [redacted] back to first grade. She simply does not know how to spell. A good reading for her is "See John Run". It is a very good book for a first grader.

Now with all of her mistakes. The company I was working for Was STS Line Maintenance at the [redacted] in Washington, DC and not for [redacted] nor [redacted]. Correct her paperwork. Include the correct spelling of John.
