I am rejecting this response because:
Good Day,
Thank you very much for your efforts
and time and I will respond to DrT***’s letter since obviously we have a
disagreement
I was charged in full in advance on
September for the whole treatment including final treatment, and permanent
crowns, in amount of $on *** card as charges out of my pocket and
in the same time estimated charges for full treatment-$were submitted to
Dental Insurance which anyway were denied later on as by the Dental Insurance because was considered as an incorrect treatment plan
Just days later on, using my right of second opinion, I
decided to not continue with DrT*** dental treatment plan and the decision
was communicated to the dental office on a phone conversation and followed by
face to face meeting with DrT*** on the same day- September 19,
At that time the dental office offered
me a refund of $from the initial charge on my *** account(out of my
pocket) of $with the understanding that however, the partial services
offered to me on September 15, will be addressed to my insurance company
This charges for partial treatment were never submitted to
the Insurance company and next step I tried disputed the charge with ***
company
Surprisingly, in *** resolution letter (attached to
this) Galleria Dental Arts office claims that “all services were rendered” and
that blocked my dispute resolution with DrT*** dental practice
The statement to *** was at
least incorrect-DrT***’s Dental Practice provided partial services and not “all
services rendered”
DrT*** pretends that I agreed to be
charged $500.00xtooth on 09/15/when actually I agreed to be charged just
for services rendered on 09/15/2016-dental bridge removal, one decay removal
and temporary bridge- based on regular prices practiced by Galleria Dental Art and
very important, those supposed to be sent first to Dental insurance company!
I received $refund and I was
waiting three weeks for Dental insurance response for the partial services
provided, just to find that no partial services were submitted to the
Insurance
I called the office and I was told that
they will try to submit the charges for the partial services to the Dental
Insurance
This did not happen and this is the
reason why I tried to dispute the charge through ***
Also, in DrT*** response, he
mentions “provisional crowns”This is incorrect, is just one provisional
bridge”, not independent provisional crowns
I was supposed to be responsible just
for out of pocket charges after Dental Insurance will decide what are the
amounts covered for services -real services! provided to me on 09/15/
Dr T*** Dental Practice as a good and
fair policy to its clients supposed to full refund my initial amount of
$to my *** account and submit the partial service charges that
he provided to Dental Insurance Company.This dispute is not only about the amount,
but about the practiceI was treated partially, I was supposed to be charged
partially through my Dental insurance.

To whom it may concern,This notice is in response to complaint concerning the treatment rendered. Mr. [redacted] presented to our office on 9/14/2016 as an emergency patient with pain in the upper right quadrant. Our exam and radiographs have shown a defective restoration and a failure of his endodontic...

treatment of splinted restorations on teeth #3,4,and 5, Based on our diagnosis we saw a need for a consult with an endodontist who recommended removal of the splinted restorations and excavating the decay to re-evaluate the area. On 9/15/2016 Mr. [redacted] presented to our office again for removal of the existing restoration and removal of decay and the fabrication of a new temporary restoration replacing teeth #3,4, and 5. Treatment was explained in detail, both financially and dentally, consent form was signed in the doctor's presence. Existing crowns were removed and the decay was excavated and new provisional crowns were fabricated and placed. On 9/9/2016 Mr. [redacted] had a change of plan for the direction of treatment. He wanted to have the teeth extracted and implants placed. We spoke to him at length about the amount of work we had already performed and we agreed we would only charge the amount of our provisional crowns, $500/tooth. We refunded $495 to Mr. [redacted] at that time. We believe that considering the circumstances, that he had a good amount of time to consider the treatment options presented by our office and the endodontists before continencement, since no treatment was rushed or pushed on Mr. [redacted], and since we have adjusted our treatment plan to only charge for the treatment rendered on 9/15/2016, we consider this matter closed.Sincerely, T[redacted] DDSGalleria Dental Arts
