I saw Ms [redacted] for the first time In my clinic on March 13, Because of the applicable federal andstate privacy laws I cannot disclose any Information about the visit, including facts about her medicalcondition, the proposed medical tests or procedures, If any, and/or prescribed medications, if any.However, should Ms [redacted] authorize me in writing to disclose the otherwise protected information toRevdex.com, I will honor her wishes and will provide you with a relevant copy of her chart along with all billingnotesThis Information will dispute many of the allegations, will establish that Ms [redacted] receivedappropriate care, and will establish that appropriate billing codes were usedThe information will alsoestablish that Ms [redacted] was never harassed by this office.Ms [redacted] was seen in my clinic on March 13, Per my office manager and biller, her Insurance wasbilled on March A response from the insurance carrier was received on March 18, Aftera reply from the insurance carrier, it is our custom and practice to submit a bill for any co-pays ordeductibles to the patientAli of this information is electronically documentedIn her complaint Ms[redacted] indicates that she received the bill for her office visit on May 4, 2017.My office will typically call patients about medical matters as well as billing mattersWe do not harassour patientsIt is our custom and practice to call patients to remind them to do labs and any other teststhat are recommended.After I learned that Ms [redacted] was unhappy about the billing, our office attempted to reach her In orderto resolve the issuesShe was called on approximately May 8th and May 9th Her answeringmachine did not allow us to leave a messageIf we were able to leave a message or if she returned ourtelephone calls she would have learned that we were and still are willing to work with her to address thebilling matter to her satisfaction.I am a Blue Cross preferred providerI coded Ms***'s visit based on Blue Cross allowable chargesIunderstand that Ms [redacted] is upset about the cost of health careMs [redacted] has not paid for herconsultationIn order to amicably resolve this matter, I will not pursue payment for my time and effort.She will not receive any further bills from this office regarding her office visit on March 13, 2017.Sincerely,Michelle R [redacted] MD

Per our store policies, we require 1 business day to process orders. The order was placed after business hours on Thursday. We were closed on Friday, and the order was processed and shipped on the next business day. Ground shipping to you, even if it was shipped the next business day possible...

would still not have reached you on the day you requested (4 business days to your location). Expedited shipping would have been required to reach you in time to meet your date, which you did not inform us was a requirement of your order. Should you have informed us, we would have informed you that expedited shipping would be required.We upgraded your shipping at our cost, and offered you even quicker shipping with us paying the difference, but you declined this. Based on our experience, I believe we offered service that is as detailed in our store policy, and went above to upgrade your shipping at our cost.Please let us know if you have any questions.
