I am writing in response to a customer complaint about receiving a copy of her records. In preparation for Dr Angle's retirement a notice was placed in the newspaper in *** *** ** for over a month that the location would be closing and the date of closing and that patients were
welcome to pick up records during that time. Since the actual closing, Dr Angle has moved all patient records to a locked facility at his home Therefore, I do not have direct access to these records I have given him the patient name twice and that she would like her records sent to her home Most recently I reminded him last week Due to his retirement, he has been on some extended vacations and also ill ..I am doing all that is in my control to do for this lady to receive her records, but I do apologize is it taking so long Sincerely,*** ***Angle Eye Center
