In dealing with Protected Health Information, we are limited to what can be disclosed. Much of the complaint contains generalized statement about how the individual reviews medical practitioners and is not specific to her experience with our office. There is no indication that a
work related injury occurred or that she has notified her employer that she was injured or has medical issues related to work. If she needs a form completed, she can present it to the officeIt will be completed accurately and truthfully by a medical practitioner according to the information in the medical recordThe patient cannot dictate what he/she wants written on the form.Sincerely,

I told Dr. K[redacted] what was going on with my type of job, she got some papers that I wrote on May 23 and May 24 2014. I called the office to get an appointment on May 5 2014, I was with severe fatigue and on May 26 or so I got another appointment with her. I did it not that her position was a assistant not a DOCTOR or PHYSICIAN, I did it not the diference.
that office has my records and I told them, I have proof that I have neck misaligment and all of this is because my job. On May 24 2014 I stopped working and I need that letter from them. It's just the thruth.
I need that letter saying my conditions and caused by my work related.

Review: I am a former patient who used GIMG as my general practitioner for approximately three years. In December 2011, I had a routine visit for nursing school admission tests. The tests were covered by my insurance with the company I worked with at that point. In January 2012, I quit my job for nursing school and therefore changed my insurance provider. In March 2013, over 15 months after service, I received a bill from GIMG for the routine tests done and an erroneous charge. GIMG claimed that my insurance did not cover the tests or that I did not have insurance. They did not know what the exact reason was and didn't know who would. After contacting GIMG and my former insurance provider, it was established that GIMG never sent the claim to my insurance. I gave the fax number to GIMG to re-submit the proof of claim to my insurance company and despite two follow up calls, they continue to haggle me for funds that are not my responsibility due to their inability to conduct basic business practices.Desired Settlement: Resolution of zero balance due and record on Revdex.com for their poor business practices.
