In response to the complaint made on the Revdex.com website, I have had a opportunity to speak with my nurse management team and review the chart and electronic chartFurthermore, I did meet and speak with the patient on separate occasions while patient was here at Meridian CenterDuring these conversations, none of the mentioned complaints were ever relayed to mePersonally, I was in patient's room twice and both times I noted zero concerns with the housekeeping of the roomPatient did spend the majority of his time in the room and did also have meals in his room To address the housekeeping issues; resident rooms are cleaned daily and linens are changed on bath days, as needed , or upon resident requestResident rooms are swept, mopped, dusted, bathrooms and vanities cleaned dailyResidents do have the right to refuse any services we provide at any timeRounds are conducted weekly by Administrator, Director of Housekeeping and Director of Nursing to address housekeeping concernsTo that, minimal housekeeping concerns are noted, Secondly, housekeeping is address monthly in our "Residential Council" meetingTypically, 15-residents attend the meeting, to date there are no issues noted for housekeeping Our medical director did see the patient while he was here at Meridian CenterPatient was relatively independent and minimal clinical needsSkin checks were getting performed xupon admission and within the first hoursOther than a "small abrasion near the great toe" there were not any skin issues noted to relayed to nursing departmentResident independent such as this case, do have the right to refuse bathsBaths are offered weekly 2-times per weekChart notes do reveal several times patient refused a bath I do request if family is upset or has certain issues needing addressed, they please bring these things to my attentionNeither patient nor family ever brought issues to my attending during patients short stay at Meridian Center

I tried to respond to this complaint but I guess it didn't go through. I have pictures of my dad's feet that were take on the day that I discharged him from this facility and they were are open wounds on his toes. The nurses there never took the time to even look And on the housekeeping part, they lie, I had to go to the nurse station to ask them to wash his floor as I slipped on a noodle in there. I put stuff in the trash days prior and it was still in the trash. They DO NOT clean the rooms nor change the bedding

To address the housekeeping issues; resident rooms are cleaned daily and linens are changed on bath days, as needed , or upon resident request. Resident rooms are swept, mopped, dusted, bathrooms and vanities cleaned daily. Residents do have the right to refuse any services we provide at any time. Rounds are conducted weekly by Administrator, Director of Housekeeping and Director of Nursing to address housekeeping concerns. Our medical director did see the patient while he was here at Meridian Center. Patient was relatively independent and minimal clinical needs. Skin checks were getting performed x2 upon admission and within the first 72 hours. Other than a "small abrasion near the great toe" there were not any skin issues noted to relayed to nursing department. Resident independent such as this case, do have the right to refuse baths. Baths are offered weekly 2-4 times per week. Chart notes do reveal several times patient refused a bath. 
I do request if family is upset or has certain issues needing addressed, they please bring these things to my attention. Neither patient nor family ever brought issues to my attending during patients short stay at Meridian Center.
