My Application was sent past when I was toldThe state (idph ) told me they changed dates on my application using white out with out my knowledgeI had to pay to get my prints redone because this business would not help meThey have to get your paperwork with in days because that is how long your prints are good forTook days to get my cardThey have yet to refund me for there mistakes or even apologizeThey blame the state the patients and the doctorsThe state had all my doctors paperwork within a week but my application which was done Dec 30th the state did not receive it until feb 28thWhen I informed the company of this and asked for proof they sent me a text saying they were done and left me hangingThe state informed me that this has happened to others and to file a report to the illinois attorney general and Revdex.comI did and the state is investigating themAlso the owner is a felon his name is Eric D *** criminal records are publicJ
