sign.We also have no problem giving the patient her dental/medical records and x-rays but we need a sign release stating where she would like them sent toThe patient does not want to sign.I offered the patient to send me an e-mail with her signature since she did not want to sign our dental/medical formI still have not received that.The patient has called several times to come in but then changes her mindShe went to *** and sent us an e-mail that because of there care she no longer needs oursI reached out to the patient on November 23,2016.and she sent me an e-mail today November 25,that she will reach out to me at a more convenient time

My name is *** ***I am sending this message to request that the investigation be reopenedI apologize for not answering soonerI was having health issues, which my primary care provider can validateIt is true, that I stopped trying to get my medical records from Gentle Dental. That is my doctor did not require the records to treat meI was never offered a " refund release", as Gentle Dental claimsI would challenge their statement and ask for a copy of their standard "refund release form." I have provided evidence below to support my claims. I was only offered a "release of records " form, which I did not sign because of its conspicuous languageDue to the fact Gentle Dental already breached our contract and asked me to sign an invalid form stating, "I was warned of all negative consequences of removing my braces prior to its a removal" and " I release Dr*** of all negative consequences," I proceeded all forms with caution afterwardsThe release form emailed to me stated I agree to the "information given to me verbally." When I asked what verbal information they were referring to, they would not respondI have a screenshot of our emails attached to this emailI also noted that their business's name was different on this form than the one in my contractIn my contract, they are referred to as "Gentle Dental." In the release form they asked me to sign, they refer to themselves as "Gentle Dental Associated and Gentle Communications,LLC." They refuse to refund me until they can protect themselves legallyWell, I'm trying to protect myself as wellDuring the duration of receiving outside medical care, I discovered that my orthodontist prescribing braces to treat jaw grinding was both unethical and impossibleMy teeth are chipped and cracked as a result of the orthodontic treatment, and they refuse to refund me. That's the least they owe me. I am contacting Revdex.com for help retrieving the refund I'm owed. Screenshots of the emails between a Gentle Dental manager and myself: to prove 1) I attempted several times to retrieve answers I needed prior to singing the release form2) I sent her photos of my teeth hoping she'd help me by scheduling me to see my orthodontist3) I gave up trying to find a resolution on my own November 18th, after a month of no progress from Gentle Dental. The insurance plan he has is a significant discounted fee for each procedure.,which is set by the Insurance companyThose fees per his plan are paid at the time of serviceWe treat the needs of the patient not the Insurance company. The patient came to us as an emergency and chooses the procedures he wants to have doneWhen the patient had the treatment done he did not return back for a few monthsThe tooth had fractured and he needed another procedure in which he signed a consent formPatient had the option of saving his tooth or having an extraction.No additional discount will be givenHis balance remains $

We would of liked to have resolved this with the patient but she has decided to take it to another level..At anytime we can revisit the concern

Even though there are some inconsistencies with the response as they said that the employer should go over my benifitsThis is true, but the business should mention this when a patient asks for an estimateThey should not provide a estimate, ASSUMING that the insurance will cover itI agree to the terms ONLY if I owe them $and no other amount is owed whatsoeverIf the business can confirm this, I can then close the complaint
