[redacted],The complaintant on referral from her Neurologist, was treated at our office on July 27, 2015.A claim was filed with her insurance carrier, [redacted], for the services rendered. [redacted] requested a copy of the medical documentation for this service in order to process the claim. The complaintant refused to release the requested documents to the insurance carrier. By her report, she called them and told them they could not have the information and insisted on knowing the credentials of who would be reviewing any of her information. Lacking the requested supporting documentation, [redacted] denied the claim for the services rendered; as stipulated to in their contract with the patient.
