Geriatric Care Managers of WNY, LLC is a Care Management organization, we are not physicians nor are we attorneys' and do we represent ourselves as suchOur role as Care Managers is to assist families and clients to understand the current healthcare situation of their loved one and assist them in navigating this very complex systemThe initial referral was made to our organization through the ad ministration of the Erie County Medical Center as the result of the action taken by the Erie County Deptof Adult Protective ServicesOur role was to advocate for Mrs*** and our client so that she could be integrated into the care of her husbandWe met with the physicians at the hospital, the nursing staff, the social work staff as well as with their attorney in order to facilitate this process, as well multiple in person and phone conversations with Mrs*** We were able to able to accomplish our immediate goalWhen it became clear that Nursing Home placement would be required we assisted Mrs*** in understanding this process and how it is paid for. At the time of discharge from our services, and after we had organized much of the necessary documentation for a Medicaid application it was identified that ECMC would assist with this processWe do assist families with Medicaid applications on a regular basis and our fee is $2000.00, which would have been an additional fee, but not needed in this circumstance.I do feel that we represented our client's best interestShould you have further questions about our services please feel free to call.Sincerely,*** ** *** ** ***Care Manager
