OSW has scheduled to come out and view my home and concerns on June 5th at 9am. The issue is unresolved as of now. Attached are photos from May 19 and 22, 2015 of mold in the garage and unrepaired cracks in the garage floor.

We met with Ms. [redacted] and reviewed desired work with her. The additional work is some additional drainage to ensure full protection of the contracted area. We are also assisting Ms. [redacted] with any damages resulting from a ruptured waterline that occurred during the installation of the job.
