As noted in the last response, I have contacted our IT consultant for our electronic health record. After hearing back from him, I was recently able to compress the files down to an email friendly size. I have already sent these files to [redacted], who now has possession of her entire clinical record and receipt for services provided. partners about this and not the complainant.  Mr. F** was under the impression this was a minor leak.  It turns out there were 18 leaks and they were having the property inspected for Section 8.  The complainant had other people in doing work that weren't licensed.  He couldn't do their timeline.  When he went back, the complainant asked that he leave while the inspection was going on. That was fine. He called back, Mr. F** showed up and then was asked to leave again.  Mr. F** indicated he had other jobs he had to do and things turned.  The complainant called him off the job and said he wasn't paying for the work that was done.  Mr. F** said he wasn't leaving because there was unfinished work and wasn't paid for the work they did.  He opened gas lines only for the few lines he worked to make sure it worked.  He is a state licensed contractor and is reputable, fair and professional.  He isn't pursuing this customer for the lien and will have no further contact with the complainant.

[redacted] has the option of waiting for the last part of her chart, at no additional charge -- or she has the option per Indiana Code 1-71-3 of paying per page to have her records printed and mailed. The cost, again per Indiana Code 1-17-3, is as follows (in addition to the processing fee for those already sent electronically): first 10 pages no charge, then fifty cents per pate is required through page 50, and then pages 51 and higher are billed at twenty-five cents per page. The client is responsible for actual mailing costs, and a twenty dollar fee if certification is requested.We are in process of fully resolving this complaint and are happy to do so. However I would like to note that [redacted] still owes us a substantial balance for care provided, even after we wrote off a considerable amount due to financial hardship, for which she agreed to pay monthly until paid off. When we don't receive payment from our clients, it makes it exceedingly challenging to pay administrative staff to process requests such as these. We do depend on financial reimbursement for services rendered to continue extending care to our clients in an efficient manner.

procedures as outlined for all AGS personnel.  I apologize profusely to Mr. [redacted] and anyone else who may have been at his home. Unfortunately, both Mr. [redacted] and the salesmen he was with were both new to the company and though still in training were anxious to try to make a sale on their own.  The salesman with Mr. [redacted] mentioned he had seen the homeowner in the back yard and so without thinking Mr. [redacted] went up the drive to try and say, "Hello" and was afraid Mr. [redacted] might not hear the doorbell.  My understanding from Mr. [redacted] statement is that he did not hear nor respond to the "Hello".  Regardless, Mr. [redacted] should not have been up the driveway, and this has been reinforced in both sales meetings since the occurrance. Again, Mr. [redacted] has my sincere apology along with a thank you for bringing this to my attention.  It is definitely not the AGS LLC method of operation. Review: Roofer put roof on shed. I specified how I wanted it put on and he did it his way. Now the roof leaks. I called three times, spoke to [redacted] once and never received any follow-up call nor has anyone come out to redo the roof.Desired Settlement: I want Golden Roofers to come out and repair the roof so it does not leak.
