I am rejecting this response because: to this day my paper still have not been submitted..gvhc state that there are no physicians that are on site at that particular office..this is why they should have an online account with the disability offices to process these forms in a more timely and efficient mannerI have been undergoing extreme stress and financal hardship due to this matter..which is not helping in bettering my condition in which I was diagnosed with

After further review it has been noted that Patient dropped off her form in the office on 08/11/not 08/07/15. This location has a turn around time of days or less. Her forms were mailed 08/20/to the State. Patient submitted a complaint on 08/20/to Revdex.com. Our management team has contacted Patient with updates on her form status. We also offered to advocate for patient as a hardship with the State to process her form ASAPUnfortunately the State stated that they had not received her information as mail takes 3-days to arrive to them this was on 08/25/15. State suggested that GVCH follow up with them 08/28/to allow more time for document to arrive.We will continue to work with the State for a resolution to this matter. We will also communicate with our Patient as soon as we have more information.Thank you

I am rejecting this response because: I went new dentist nothing was done, I want golden valley  explain why they charge me for I want my refund back , If they don t want work with me now I will open court I let the judge decide.  I have proof what they did was wrong.

Review: I went to the Golden Valley location on 12/4/15, a Friday, because my ear was bothering me. I made an appointment earlier that week. I went to the appointment and sat for almost two hours. I finally saw got to see a physicians assistant and he diagnosed me with an ear infection and prescribed me three medications. After my appointment he was suppose to send my prescription to my pharmacy so it could be filled. He failed to do so. I waited and called the office on Friday, a few hours after my appointment, to remind them to send it. Saturday they still had not sent it so I called again and was told that their offices are closed on weekends and I had two options, I could call the pharmacy and ask them to call the doctor and request my prescription or go to an urgent care and be seen again. I called my pharmacy and was told that it wasn't possible for the pharmacist to call my doctor and request my medication. That my doctor doesn't answer the pharmacies calls on the weekend. I decided to wait until the following Monday morning to get a hold of my doctor. I was told he isn't in the office and basically I'm out of luck until he gets back. The office assistants were rude and not helpful at all. I made the appointment and did what I was suppose to do but Golden Valley wasn't trying to be helpful at all. I'm sick. All I want is my antibiotics and some common courtesy. They can bill my insurance but not even send my prescription? So now my only option is to be seen again. It was a waste of my time and Medical's money.Desired Settlement: I would like my prescription filled or I would like them not to Bill my insurance. I would also like an apology.

Review: I'm having an issue with medical records at this office. I have given them 4 sets of forms, two which are fmla and disabilty forms about 3 weeks ago and they have failed to complete these forms. I am with no income and I am on the verge of losing my job due to them not compeleting these necessary formsDesired Settlement: For them to find a better solition as to how these important forms are being handled, in hopes that other people would not have to suffer financle lose do to their laziness

The worst place to go to if you want to deal with professional people the employees treated me horrible when calling to ask a question I get hung up on told I can't speak to a supervisor and told not to call back when I just needed to ask a question about my prescription they put me on hold for half an hour then hung up on me and this has happened almost every time I try calling
