2015Complaint Case #: [redacted]Consumer: [redacted]To Whom It May Concern,I am in receipt of Mr. [redacted] Revdex.com complaint. In researching his account, Mr. [redacted] did manually sign a PT EFT Agreement agreeing to the terms and conditions of his agreement. The PT Agreement continued on a month to month basis after his initial term ended and he was required to give a 30 day notice per the terms of his membership agreement to cancel his PT EFT Membership. We did not receive a cancellation notice from Mr. [redacted] and therefore his PT EFT Agreement continued on a month to month membership.I have cancelled his PT EFT Agreement. However, due to the 30 day notice that Mr. [redacted] is required to give per his membership agreement, he does owe the past due balance of $455.99. This balance consists of this Personal Training dues for July 24, 2015 & August 7, 2015 dues for $220 each bi-weekly billing cycle and his membership dues of $15.99 for August 21, 2015 bi-weekly billing cycle.Until this balance is paid, Mr. [redacted] may not access the gym as he has a past due balance on his account. He may not cancel his membership at this time as he signed a 12 month agreement and he is in obligation until February 18, 2106. I have set his membership to cancel at that time.
