Faxreport, it was sold in South Carolina where it was serviced by the original and only owner until it was traded inat Orange Park Dodge on 01/04/2016 with 42,041 miles on the odometer which agrees with all the carfax servicerecords.  We sold the vehicle to Mr Glover on 3/23/2016 with 42,062 miles on the odometer.  It of course had about 21 more miles on it from test drives, etc. The odometer, has not been altered or tampered with in any wayby Orange Park Dodge.                                     ...⇄ Thank you,                                      ...⇄ [redacted]                                   ...⇄ Manager

First time utilizing the ER, room literally has needles on the floor and garbage. Our 5 year old son needed X-rays for his chest due to him coughing up phlegm and while the X-ray was being performed the actual way the X-ray tech yelled at my 5 year old child asking him if he was ready for an X-ray the professionalism at this hospital as well as osha standards should in my opinion shut this place down. As a veteran I would rather have field triage than use this facility st least I'd know the procedures for keeping a wound clean. Pathetic and absolutely worst.

This email is regarding the complaint [redacted] ([redacted]) received most recently dated 3/2/2015 and received by us on 3/6/2015.  On behalf of the organization, I apologize in the delay in our response.  Our compliance officer position is in transition and therefore a timely response did not occur.
 
Investigation for  [redacted] ([redacted])
I spoke with our Director of Corporate Patient Financial Services and found the following.
Patient was treated in our Emergency Department 7/9/2013-7/10/2013.  When our billing verified insurance with [redacted] it came back as ineligible and [redacted] was a secondary insurer, therefore to prevent out of pocket expense for the patient Blue Cross/Blue Shield was billed.  We have notes in our system of patient contact with us with a request to bill [redacted], however we cannot validate that the patient contacted us 5 times.  We have billed [redacted] as requested on 3/2/15.  When we receive payment from [redacted], we will refund  her $783.45 to the paitent.
