And saying they won't put it back until I sign an agreement I am not the person doing wrong I kept my wordHere is the claim ID for the money fraudulently taken from the FSA: [redacted] Amount $Merchant: [redacted] Family Dental It was not a mistake on my part, I specifically told them not to use the card on February 20th and they still did thinking they could force me to pay the money in full at a later date If they were really willing to work with mehave them reverse the money to the [redacted] Group as that is the honest and professional thing to doAs I stated before, this is not my money they are takingI will continue to make my monthly payment as agreed to with Dr [redacted] and his wife *** My word is good and sincere as they have already received my first payment of $for April Regards, [redacted] ***

This is in response to the complaint that was filed on the lobster tails at Gordy's County Market. First of all, we were incorrect in the packaging of our lobster tails. They were in our ad as a oztail at $EA. , However, they were
packaged as both single tails and double tails and both had signage that said $ea. On that same sign I do believe there was a ozprinted, but still very misleading. This item is not on sale anymore and the complaint was actually on the last day of the sale, however- we have corrected the problem in our pricing and meat departments, where as if we advertise a lobster tail at $each the signage will be for single packed tails. Going forward any multiple packaging will be signed as such- ( Example: $for a ct package. ) We have contacted the customer by phone and the problem has been resolved as we told him the same thing, and it was our fault the way this item was signed. The level of customer service this customer received by the store manager was not acceptable to our standards and we will be having a very strong conversation with this individual, to thoroughly explain our standards of customer service by our Director of Retail Operations. We truly apologize for the way this customer was treated

On 8-30-implants were placed for the amount of $2136.00, patient paid $700.00. The abutments place on 12-17-for the amount of $3813.00, patient paid $300.00. Dr*** did a professional courtesy adjustment of $2759.00. Once the bridge was seated for the final amount of $2890.00, Patient paid $on 1-20-14. As of 1-20-all the work has been completed. Patient also stated she will pay $on 2-20-
2-20-Patient had an outstanding balance of $1595.00, for work that was completed. Patient made an agreement to pay $of the balance on 2-20-14. Patient did not honor her agreement, she stated she can't pay the $1400.00. Then she gave us a card # over the phone to charge only $
2-27-14 Patient called and was requesting her *** to be refunded. Advised patient we will be more than happy to, but we will need another form of payment. Patient stated she wanted a bill and she will call us back
3-17-14 We called the patient to set up a refund for the $with an agreement plan of the remaining balance of $1595.00. Patient said she didn't have a card available and once again would call us back when she wasn't at work
3-26-14 Sent email with the payment plan attached, needing her signature, which would allow us to refund to her card. Patient never responded
3-27-14 We left a message for the patient to see if she has received the email, once again, no response
4-7-14 Revdex.com Letter received
In response to the 'Fraud' accusation, our records show a charge of $on 8-30-13. Mrs*** was present at time of this charge. There was no fraud committed
In summary, we are more than happy to work with Mrs***, we understand it was her mistake giving us the wrong card to charge. We just need her to sign the financial agreement to pay her balance of $1595 for the services already completed

The statements made are completely untrue In their first response they said that: "the Fraud accusation, our records show a charge of $on 8-30-13. Mrs*** was present at time of this charge. There was no fraud committed." Now they are saying that I called and said I was not going to pay, but at the same time I gave them the card number? When I was in the office paying the $I mentioned that I might use my *** card for my next payment When I called on the 20th, that is when I told them the card was not eligible for the 2013, so I would make payments as arranged by Dr***They went ahead and took $from the *** anyway, since they have the number on file from previous transactions If I had approved the charge, they would have sent me a receipt as they normally do I did not receive a receipt from them, because I did not authorize this and they know itI called several times to get it reversed and when they wouldn't that is when I talked to Ms*** ***, Dr***s wife and she asked me if I could make $a month payment I said yes and have made a payment for April the next payment will be made for May I kept my verbal agreement with Mrs***.

Patient gave her word to pay $of her $balance on 2-20- On 2-20-the patient called to give her card number over the phone stating she wasn't going to pay, breaking her word, the $1400, instead she paid $ Mrs*** gave us permission over the phone to run the card for the amount of $to charge
On 2-27-she called back and requested that we refund $ We have stated multiple times, we are more than happy to refund the card she gave us as long as we receive a signed Financial Agreement, since the verbal agreement was broken on 2-20-
We did receive a money order for the amount of $325, which has not been deposited We are waiting for the agreement to be signed
On 4-14-we left another message to say thank you for the payment and will not deposit it until the agreement is signedWe have spoken to doctor's wife who originally set up payment plan and she is in agreement with needing a signed financial agreement.
