Dear Sir /Madam,I am sorry that I could not post my reply on above case ID as I was unable to copy and past and attached but I replied in separate email I would appreciate if you would add my email sent to you as my reply to Hunting Hill Dentistry I have received a reply from Huntig Hill Dentistry, please refer to my letter wrote to you as suggested by *** not to mail me a bill till my grievances resolveI was out of country from 10/29/to 04/11/I had no assess to my mail or phone till I came back I do not know how you tried to reach me.?? I was not happy when I left on 06/as I was not treated for which I visitedDr *** did not ask me to open my mouth did not even listen my problem your *** PDF as wrote by you also confirmed it's modifyI canceled my appointment text you the reason why I am cancelling being not modify by youDr *** has put a wrong code for oral evaluation ,she need to put D or Dunder 100% (0.00copay) like wise she put a wrong code for X- rays ( message from *** on 07/22/I was charged for evaluation and X-rays)) please refer to Den plan you enclosedI filed grievance on 07/22/with *** and also with *** but mail came back as *** address given by *** not correct.I know I signed a paper , I used to work in Motel and any guest like your patient need to sign a folio when they check in, if guest is not happy for any reason we were not showing his folio he signed , we always feel customer is right and to make him happy though he signed a folio we use to give discount or next stay free with apologies letter so he will continue to visit our motelI am sorry that while going through your review you are taking advantage of patients signed at checked in ,who came with full trust and confidence that they are treated well and will be happy after taking treatmentI would have been appreciate if you fill sorry for the treatment which I received and would have investigated for my concern and issue.Dr *** need to change a code from Dto D , I am still waiting for outcome of my grievance I would appreciate if you would take out derogatory mark from transunion as my issue is not yet resolve .I copy and paste letter I mailed to Huntig hill , not to Bill me till my issue is resolved as advised by ***.Case ID: ***

To Whom It May Concern This letter is in response to the complaint made to the Revdex.com on behalf of *** ** ***. Mr*** complaint alleges that our office fraudulently collected payment for treatment, treatment that Mr*** states is completed. When
the patient originally presented for a consult with Dr*** *** on date of service 02/07/2017, he was given an estimate for his upper arch only of $8,025.00, this included estimated insurance payment. The patients original amount was $11,(please see attached). Mr*** agreed to the amount and consented to treatment which began on 02/15/and was completed on 09/05/(this evidence is available in the patients note history, however; due to HIPPA constraints, our office can not release detailed clinical documentation without patient's consent). On 07/27/the patient presented for ongoing treatment on the upper arch and at that appointment consented to have a lower tooth crowned, at which time $1,was applied to the patient's account as per attached signed treatment plan for the lower teethThe patient signed a consent for treatment. On 08/14/our office received payment of $1,from the patient's insurance company towards the upper completed treatment, which was less that our office anticipated to receive. The patient's wife call our office to complain, stating she thought we owed her husband a large credit back. The patient's wife was unaware that her husband consented to additional treatment, and was also unaware that insurance had paid less than our office originally anticipated. As a courtesy to the patient and his wife, our office offered to discount the remaining lower treatment in order to alleviate future cost for the patient due to the patient's unpreparedness. Our office maintains that this patient is not due any money back, because as stated in the proposed treatment plan, "Insurance coverage is only an estimation. The patient is responsible for all treatment not covered by insurance." When I spoke with Ms*** on 4-24-17, I explained to her that refunds are processed on the 1st of the month and the 15th of the monthThere has been a check cut in the amount of $Check #***

I have reviewed the response made by the business in reference to complaint ID ***, and find that this resolution would be satisfactory to me. I will wait for the business to perform this action and, if it does, will consider this complaint resolved

Dear/Sir/Madam,I received your reply after month stating that I visited you under emergency exam,I do not understand what you mean by emergency exam as I sent several messages to *** to find out dental provider, as well as it would be under copay as I will be visiting doctor under DEN insurance I got your appointment may be after month after I called youI did not visit you in emergency so you need to put right codeYou have specified what you mean by Comprehensive exam, when patents have specific area that need to looked at and not an evaluation of every tooth.Please refer to my grievance, I visited you just to tighten or loose my 2+teeth denture under comprehensive exam and not check my all teeth. My dentist *** did not even ask me to open my mouth, I am not asking you to change the code but requesting you to correct your mistake by putting a right code.After coming to USA I learnt words which in day to day life I am using everyday ,SORRY,THANKS,and APPRECIATED. You don't feel sorry for your mistake,as you know you did not put right code, when I cancelled my next appointment your staff did not bother to ask me why?I copy and paste, for your ready reference mistake like you *** *** (Provider) has done by not putting right code and replied I received from them to apologize, rectify and to made a correction( *** *** did not do any fraud by changing code as you say)Hope you will also rectify your mistake and will put a right code (D150) like *** *** has done Thanks*** ***

I understand that your insurance will only pay for a specific code of D0150, but when you came in for you visit on 6-29-15 you present to our clinic for and emergency exam which is code D0140. Code D0150 is a code that is used when patients come into our office for a comprehensive exam. Code D0140 is used when patients have a specific area that needs looked at and not an evaluation of every tooth.  Changing that code to something different from what we have done is insurance fraud and therefore we can not change the code since we only did a limited exam on you.
