[redacted] presented himself for treatment on 7/20/15.  He signed the assignment of benefits allowing us to file his insurance and have the payment made directly to us.  He also signed his charge sheet each day that he came into our office for treatment.  Per his insurance company...

he had a $3,000.00 deductible that was not met.  That means that until he meets his deductible, he is responsible per his insurance company for the amount that they deem is due.  He even brought to our office an Explanation of Benefits for his services that he received as he had a question about a company we referred him to for testing.  He did not seem to have any problem with the amount that his insurance company said was due to us, but he never paid it.  He had $5,950.00 worth of services. Since Dr. [redacted] is an in network provider, he wrote off $4,081.51.  There were credits made to his account for using a groupon which is handled by the New Health Centers of Texas, and he also had credits that were made from the [redacted] that is run by the independent massage therapist that work out of our building.  The only payments he made to our office for all of this care was $270.00.  He does owe the $1,318.49 per his insurance company.
