protocol we invite our new patients for that week for a doctor’s report and to go over their x-rays and treatment planWe are a corrective care chiropractic office and this a standard protocolIn the doctor’s report we go over the importance of health and spinal correction we also encourage family to attendThe doctor went over her x-rays with her and the treatment plan that he recommended to correct her spineThe treatment plan is attached for your referenceIn this case the recommendation was for x a week for weeks for visits over months of careThis treatment plan includes exams at both weeks and weeks along with re-x-rays at and weeksThis is to show both doctor and patient the progress that is being madeIncluded is a home care kit as wellThe total cost of $was not just for chiropractic visitsWhen the cost of the treatment plan was discussed it was simple; We went over the above recommendation of (x a week for weeks) and the different ways she could pay for it1) If paid in full we provide a 10% discount off the $2) 15% down which was $then in days she could decide on if she wanted to pay her treatment plan off in or month payments (3)or (6) $3) We offer *** ***She can apply and she if she qualifiesShe started her credit is excellent and there shouldn’t be any reasons she should qualify and to applyI didn’t have all of her information on her patient chart which I had to ask her such as SS, monthly net income and home status which she providedShe was approved well over the treatment amount and I informed her of that and asked her if she wanted to use *** *** to pay for her treatmentAlso informing her that we pay the interest for months(when we provide this service in order for our patients to receive months’ interest free we are charge 13.5% off of the charged amountSo when she paid for her $we were charged 13.5% off of this amount) Our patients receive a discount when enrolled in a corrective care plan or a wellness planOur standard cost for adjustment is $unless enrolled in a corrective care or wellness plan then the discount can varyIn *** ***’s case she was being charged $However, when she discontinued care and was not going to enroll in a corrective care plan her per visit went up to $The address the patient provided us on the patient intake form was fill out by the patient herself who left out her apartment #When *** *** asked for a statement I advised her that I don’t have one for herShe used *** *** to pay for the service and they would issue her a card and a statement showing her balanceWhen Lorena called and left us a message she mentioned *** ** *** and when we called her back we provided her with *** ***’s phone number thinking that maybe she just called the wrong company and that’s why *** *** couldn’t help herI called *** *** myself and tried to advocate for her however with being the merchant they would not do anythingI was able to get her account number and informed Lorena to call them and use her account number and ask them to issue her a statementLorena had not provided us with her apartment # on her application and therefore *** *** didn’t have it either and they wouldn’t change it unless she called them and did itIn conclusion she had been charged $per visit for her visit which equals $She has been refunded $We were never trying to scam her or charge her for services that were not renderedI hope this explanation is sufficient enoughPlease feel free to contact me with any further questions, comments and or concerns*** *** Granite Bay Spine & Body *** *** *** *** *** *** Roseville Ca ***

I have reviewed the response from Dr. [redacted]'s office and appreciate the detail they've provided.  I agree to the $455.00 as stated in their response, however I have attached a recent invoice from [redacted] that does not match the agreed amount and still reflects the previous $829.00 balance.As always, I have been requesting an invoice from Dr. [redacted]'s office that I can submit to my insurance company for reimbursement.  This invoice should reflect each date of service, the medical code matching what service was administered.  They agreed to provide this to me as I had explained it was necessary or I couldn't be reimbursed.  It didn't matter to me if they sent it or not because I could easily send it myself.  Dr. [redacted] said their office wasn't set up to submit invoices to insurance companies and wait for long periods of time to get payment and I understood.  So if I could get the detailed invoice and the revised [redacted] statement to reflect the $455.00 balance, I would be satisfied with the outcome and pay my bill in full.Thank you,[redacted]

Review: I had a chiropractic evaluation appt. with Dr. [redacted]. At this time he performed a lengthy marketing sales pitch to a group of people, then individually brought us back, took an xray, and said he could correct my neck problems over time (6 months - 3 adjustments per week or 26 weeks)for a total amount of $2,665. I told him that I had insurance that covered chiropractic appts. He said they don't bill the insurance company directly, but he would issue it to me instead. I was provided an invoice for that days appt. for $82.00 & I charged my [redacted]. I wanted to pay my bill each month, however [redacted] pushed for me to use [redacted] & to see if I qualified. I didn't know what that was, but had heard of it as an alternative way of paying your bill. She said I qualified and that was it, no explanation of how it worked, no contract was ever issued, no invoice was provided other than the 1st appt. I was told that [redacted] would do everything & they would submit the invoices to me. After 2 weeks, I realized I didn't have the time or money & cancelled their services after 8 visits. I left them a voice msg telling them that I wanted to pick up my invoice and pay them. When I got there, [redacted] didn't know what to do. They had already been paid in full by [redacted]. I said how is that possible, I've never recvd an invoice. I said, give me an invoice showing the appointment dates so I can turn it in to my insurance company for reimbursement, and then I guess you would issue a credit. Who gets paid for services in advance? [redacted] had been given the wrong address from the dr. office, & because of that, they said I didn't owe a payment until July. So, if I don't owe a payment, how can the dr office charge me 13% when they never issued me a bill to begin with and there was no explanation how they arrived at the $?Desired Settlement: I want a revised invoice including the dates that I was there & credit, reflecting the correct # of appts included in a 6 month plan. This should be illegal to charge somebody for services that never took place.

3 visits per wk for 6 mo. = a total of 26 weeks X 3 visits per week = a total of 78 visits within 6 months.

Total cost of services $2,583.00 divided by 78 visits = $33.12 per visit.

8 visits @ $33.12 = $264.96 - I would like an invoice please.

70 visits @ $32.12 = $2,318.40 -

Good Afternoon, Yes, [redacted] came in for her initial exam, X-ray and consultation, which she paid $87.00 for. She made her follow-up appointment which was recommended so that the doctor could go over her x-rays with her and the treatment plan he recommended based on her x-rays. As our normal protocol we invite our new patients for that week for a doctor’s report and to go over their x-rays and treatment plan. We are a corrective care chiropractic office and this a standard protocol. In the doctor’s report we go over the importance of health and spinal correction we also encourage family to attend. The doctor went over her x-rays with her and the treatment plan that he recommended to correct her spine. The treatment plan is attached for your reference. In this case the recommendation was for 3 x a week for 13 weeks for 39 visits over 3.3 months of care. This treatment plan includes exams at both 8 weeks and 12 weeks along with re-x-rays at 8 and 12 weeks. This is to show both doctor and patient the progress that is being made. Included is a home care kit as well. The total cost of $2583.00 was not just for chiropractic visits. When the cost of the treatment plan was discussed it was simple; We went over the above recommendation of (3 x a week for 13 weeks) and the 3 different ways she could pay for it. 1) If paid in full we provide a 10% discount off the $2583.00 2) 15% down which was $387.45 then in 30 days she could decide on if she wanted to pay her treatment plan off in 3 or 6 month payments (3)731.85 or (6) $365.93. 3) We offer [redacted]. She can apply and she if she qualifies. She started her credit is excellent and there shouldn’t be any reasons she should qualify and to apply. I didn’t have all of her information on her patient chart which I had to ask her such as SS, monthly net income and home status which she provided. She was approved well over the treatment amount and I informed her of that and asked her if she wanted to use [redacted] to pay for her treatment. Also informing her that we pay the interest for 18 months. (when we provide this service in order for our patients to receive 18 months’ interest free we are charge 13.5% off of the charged amount. So when she paid for her $2583.00 we were charged 13.5% off of this amount) Our patients receive a discount when enrolled in a corrective care plan or a wellness plan. Our standard cost for adjustment is $65 unless enrolled in a corrective care or wellness plan then the discount can vary. In [redacted]’s case she was being charged $55. However, when she discontinued care and was not going to enroll in a corrective care plan her per visit went up to $65.00. The address the patient provided us on the patient intake form was fill out by the patient herself who left out her apartment #. When [redacted] asked for a statement I advised her that I don’t have one for her. She used [redacted] to pay for the service and they would issue her a card and a statement showing her balance.
