Dr [redacted] needs to explain why all charges were not submitted to the insurance.I request a clean copy of my account history report, without handwriting all over it, unless they are explicit accounting notes, and ALL EOBs for this account, and they need to be clear and concise, without figures scribbled on them that are not clearly correlated to the Account History Report.I am ordering EOBs from [redacted] Dental and statements from my Health Spending Account and these will take business days or so to receive We will continue to work on this complaint until satisfaction is reached Regards, [redacted]

After assessing the patients needs a referral and a x-ray was given to the patient to see a local endodontist in town. Patient did call and talk to our Front Office Coordinator and she informed him of the procedures to go to the University Of Louisville Dental Clinic. At that time he was told that... he did not need a referral to become a patient at UofL dental clinic. He need to go to the clinic and they would handle his needs at that time. At no time did the patient give us a contact name or number for anyone he spoke with at the University. To my knowledge if a patient wishes to become a University of Louisville patient they need to call the University at ###-###-####, or go to their emergency department and be seen. At that time if they need any additional information from a previously dentist then they will contact. After reading this complaint I decided to find out for myself exactly what the protocol is to be seen at the University of Louisville Dental Clinic and at the Endodontic Clinic so not to have this situation occur again. I was instructed after calling the clinic number that all a patient would need to do is go the dental school emergency clinic, have an exam and then get a referral to go to the Endo clinic. If a patient wanted to by-pass that process then they could call the Endodontic clinic and tell them your needs. Once they give the clinic the dentist's name and number they will contact that dentist to get the referral. They will even fax a referral to the dentist if the patient has a fax number. My hopes is that with this information the patient will go to the Dental School Clinic to have his tooth treated.

I'm attaching a new copy of the patient's account historyThe EOB's that where previously sent are the only copies that I haveNot all services are coded to go to the insurance companiesSome services are in-office services and are not required to be sent to insurance, i.ewhitening services. Hello. I have looked into the complainant's account. A thorough account audit was done by myself. I reviewed account transactions starting from the date of service 3/26/12 to the date of service 6/4/12. Insurance EOB's where included in the account audit. I discovered errors with account...
