Our office recently received a complaint from your firm and I would like the opportunity to dispute itIn regards to Ms [redacted] and her complaint we have tried contacting her regarding her wanting to cancel her treatmentIt is our practice that we receive a written day notice from patients advising that she/he would like to cancel their planWe did make an attempt in revising her plan so that she could remain doing her treatment but she did not return our callsI had left several messages advising that we do need the written notice and until we received the complaint from your office we heard nothing furtherWe have now cancelled out her plan and with the cancellation fees and the costs out of the agreement to cancel she will be receiving a refund in the amount of $This refund will be sent out todayI have attached our documentation along with her signatures regarding the proper way to cancel out a plan and the feesIf there is anything else you may need in order to quash this complaint please let me know as soon as possible.Sincerely, [redacted] **

patients advising that she/he would like to cancel their plan. We did make an attempt in revising her plan so that she could remain doing her treatment but she did not return our calls. I had left several messages advising that we do need the written notice and until we received the complaint from your office we heard nothing further. We have now cancelled out her plan and with the cancellation fees and the costs out of the agreement to cancel she will be receiving a refund in the amount of $2.64. This refund will be sent out today. I have attached our documentation along with her signatures regarding the proper way to cancel out a plan and the fees.
