the plan of care as discussed with her Rheumatologist. Her Rheumatologist ordered labs on 2/7/2017 to further evaluate her condition. Ms. [redacted] did not have her labs completed until 3/27/17. The Rheumatologist spoke with her personally, by phone, and discussed her lab results in detail. She was, unfortunately, unsatisfied with the Rheumatologist’s explanation of her labs and declined our offer to schedule a follow up appointment to further discuss results to her satisfaction in person. Ms. [redacted] also had labs ordered by her Hematologist. The Rheumatologist reviewed those labs ordered by her Hematologist with her as well. Her Rheumatologist did advise her to follow up with her Hematologist, the ordering physician, as is appropriate. She has been referred to another Rheumatologist, as requested. The medical release form has been sent to her address in order for her to receive her medical records.
