Complaint: [redacted] I am rejecting this response because:I have evidence to the contrary The test was provided and ordered by [redacted] of Guardian Healthcare CentersShe has an established account with Diagnos TechsPlease see supporting documentation My former healthcare provider, Guardian Healthcare Centers, managed my hormones, thyroid and blood pressure medicationsThey would not refill my hormones without having the necessary lab work completedI requested a blood test, but [redacted] indicated that blood work was not very accurateMy endocrinologist was NOT convinced that the test that [redacted] ordered was very accurateShe has never heard of saliva tests [redacted] insisted on performing this test, knowing that this was an out of network labShe was also aware of prior problems with this lab It was the provider's responsibility to inform the patient of the charges associated with this lab and to let them know that this was an out of network labShe said the hormone panel would be $95.00, regardless of whether it was covered by insurance or notIn fact, she encouraged me to include a copy of my insurance card I urge other to read the multiple complaints filed with the Revdex.com against Diagnos Techs in Kent, WA [redacted] has not been honest and forthright in educating her patients about the costs associated with this lab I called the nurse practitioner’s secretary only three times asking if she could help in getting this issue resolvedShe indicated that they had another patient encounter similar issues with this labI asked her to fax me a fee schedule(Please see attached) The secretary indicated that she was going into a meeting but would call me afterShe called later that afternoon and she said [redacted] tried working with the lab to get the charges reduced, but since I involved the Revdex.com and the attorney general that they were no longer willing to help me get the problem resolved [redacted] got on the phone and was very disrespectfulShe indicated that now that I filed a complaint and threatened legal action that the lab was no longer willing to work with meI called the Washington State Attorney General and they encouraged me to file the complaint A former employee, [redacted] ***, indicated that [redacted] had multiple issues with the Diagnos Techs laboratories during her employmentShe was surprised to hear that [redacted] was still using this labWe both began to wonder whether [redacted] had some sanctions placed against her, preventing her from using local in-network labs I called the lab asking the lab if they would be willing to work with me due to the misunderstandingThey were very unprofessional and kept telling me to just shut up and stop speaking over themI have recorded conversations to support my allegations [redacted] has slandered my name to a mutual friend of ours and has breached my confidentiality rights electronically, over the phone and through emailShe is in violation of the HIPPA privacy ruleHealth care providers must protect individuals' medical records and other personal health information and applies to health plans, health care clearinghouses, and those health care providers that conduct certain health care transactions electronically [redacted] has violated the HIPPA privacy [redacted] shared confidential information with me about her previous employee ( [redacted] ***) This employee posted a negative review about her work experience with Guardian Healthcare Centers on their Google Places page [redacted] indicated that she was paid under the table for about of the months that she worked for ***She was asked to leave Guardian Healthcare based on a preconceived notion that ***’s son had something to do with the theft occurrence that took place at her establishmentShe claims she did not terminate ***, yet she never hired her back [redacted] was angry with [redacted] for posting this reviewShe cautioned [redacted] to not divulge this information because she would incriminate herselfShe used scare tactics in an effort to get [redacted] to remove the review [redacted] was angry after [redacted] posted the review and immediately assumed that I had something to do with this negative reviewShe breached my patient confidentiality rights by defaming me to Mrs [redacted] and sharing sensitive information with herI did not threaten or harass Guardian Healthcare CentersI am certainly entitled to share my negative experience in an effort to prevent others from becoming their next victimI am also a business professional with a familyIt becomes unfortunate when it becomes necessary to escalate matters such as this in an effort to get assistance in resolving the issueIf the business owner had taken care of the problem, I would not be sharing my negative experience I informed Guardian Healthcare Centers that I had a long LENGTHY conversation with [redacted] last night and that she was aware of the expenses associated with Diagnos-Techs laboratoriesDuring her employment, she indicated that [redacted] was no longer going to use this lab because of the multiple issues that you had encounteredShe answered as follows: Ms [redacted] ,Please do not use this email or any other methods to contact this office or [redacted] FNP againAny requests for records should be made through the mail or by way of your medical providersYour constant emails, reviews, and slander are considered harassing and threatening as you have already been toldYou were terminated as a patient of this office due to your threats to office staff and slanderous attacks onlineWe will have you trespassed from our facility and a restraining order will be sought if needed(see supporting documentation)It becomes apparent that I struck a nerve with her, because she has now terminated me as a patient for no legitimate reasonThis is a blessing in disguiseI am now receiving quality healthcare from reputable providersI do not wish to do any further business with this establishment.Multiple family members of mine and friends have had similar encounters with this officeI urge clients to do their research when choosing their healthcare The water line was NOT done as Business responded The only thing that was done was to remove the plant and leave a holeThroughout the whole month of May, he would say he would do it another day and after that even yet another day On May 20, 2016, I texted him and said "either you do what I paid you $to do or I want my money back immediately"He then texted me back saying "I'll have planted Sunday my apologies thx". On May 22, 2016, after learning that he had not done again, I texted him as follows: "You said I'll have planted Sunday my apologies thx Yet nothing has been doneI'm fed up with your lies - wondering if you ever intended to do ANYTHINGI'm done with you and I want my money back immediately No more excuses, delays, or liesVannetta"He responded asking again for more time to which I said no, too late and I wanted my money back I heard from him again via text on Thursday, June 9, 2016, wanting to come by on Saurday if it was ok with me My response to him via text was "NO Absolutely not !!! You did not do the planting nor do the waterline I had to hire someone else to do the waterline as well as do the planting the only thing you did was to remove the old plant and leave a hole I want my money back" his response via text at 4:pm on same datewas as follows: "Ok I will invoice and refund" He has demonstrated that he's not reliable and is not truthful

May 23, 2017To whom it may concern,On 5/11/my medical secretary received multiple phone calls from the complainantexpressing her dissatisfaction with the billing department of an outside ancillary laboratoryservices providerAlthough I was in the middle of a busy clinical schedule I attempted
tocontact the lab on her behalf because, I could sense her frustration level due to her tone ofvoice and the urgency of her repeated calls.The billing department staff were willing as a professional courtesy to me, relook at herconcernsThey requested that she send in her explanation of benefits letter from herinsurance and see if any reduction of her bill could be doneThe complainant was informedverbally and in writing that she is responsible for all outside lab chargesShe was alsoinformed that most insurance companies will deny saliva hormone testing and wasencouraged to pay the cash price to them in lieu of submitting to her insuranceThecomplainant insisted on attempting to submit the charge to her insurance and it was appliedto her out of network deductible.Fifteen minutes after I spoke with the lab, they called back to say they would not be willingto assist her at reducing her bill due to her repeated calls and emails threatening legalretaliationI attempted to contact the complainant about this and she denied threateningthem, but agreed she would try and speak with the lab to see if a mutual resolution could bereachedThirty minutes later she called my medical secretary and stated that she would begincomplaining about me and my business online until her bill with the outside lab was resolved.Since 5/11/she has continuously threatened and harassed my business online and writtennumerous slanderous statements about me personallyI have included them for your review.I am a busy professional with a large familyThis has been an extremely unpleasant andunwarranted response from a long time clientAny assistance you can offer to minimize hercontinued attack on my business and reputation would be greatly appreciated.Sincerely,*** ***, F

The I do floor plus people wired the Jacuzzi GFI into the switch box in attic which was wired into the light switch and that caused the problem since the light in the attic was turned off and that then turned off the power to the JacuzziThe GFI was not anchored down alsoNothing was up to code.I paid $to the contractor to put in an opening into my closet and rewire the GFI and anchor to a support beam Easy and safe access if a power outage occurs.I have heard from *** quite a few times that he would be down on weekend to do the correction Lies and more Lies He can refund me the $that I had to pay for his corrections.
