In regards to the [redacted] case, we take our patient’s complaints and concerns very seriously We always try to accommodate our patients and strive for great customer service to ensure that they are not burdened with the financial aspect of receiving their dental treatmentWe also do strive to serve each of our patients with the highest quality of health care experience possible and take pride in committing to each patient’s oral health [redacted] was first seen in our office on 2/21/At this visit, she had a comprehensive oral examination by Dr [redacted] [redacted] was diagnosed with type II periodontal disease with scaling and root planing (SRP) in conjunction with a locally applied antibiotic (Arestin) being the best gum treatment for the diagnosisDr [redacted] also treatment planned three fillings on teeth numbers 2, 15, and due to decay [redacted] was presented a phased treatment plan for both the SRP and fillings at this appointmentAs part of Dr [redacted] ’s philosophy of care, the first phase of treatment, SRP, was presented to her which included the locally applied antibiotic (Arestin)This is important in continuing to help kill the bacteria even after the SRP has been completedFinancing the antibiotic was a concern for [redacted] , and was thus removed from the initial treatment, with an expectation of adding this to one of her next follow up recare appointments After removing the Arestin from the initial treatment, her expected out-of-pocket cost came to $with insurance covering the remaining portionA treatment plan and payment agreement was signed by [redacted] stating she would pay $at the time of service and the additional $in two monthly installments A treatment plan and payment arrangement for her second phase of treatment, which included the three fillings, was also agreed to by [redacted] Her expected out-of-pocket expense was $with her insurance covering the remaining portion [redacted] was seen in our office on 4/25/to complete this treatment In regards to [redacted] ’s dispute over our fees, our full office fees are usual and customary and are comparable with other dental practices in the areaA benefit to her with having a PPO insurance carrier, Premier Access, is that we are obligated to charge her in accordance to their fee schedule which is much lower than our full office feesWe did, however, notice an error in our system which did not have the correct insurance fee schedule attached to her accountThis resulted in her patient balance being more than what it truly isAfter reviewing her account and making the necessary adjustments, her true patient balance was lowered to $ In our error and as a courtesy to [redacted] , we are going to adjust her balance down to $0, as well as removing her from the collections agencyBy doing this, she will not pay for any collections fees, and this will not appear on any credit report for [redacted] We sincerely apologize for this situation and appreciate the opportunity to improve in this matter Sincerely, Kokopelli Family and Cosmetic Dentistry [redacted] ***
