Dear Revdex.com-
We feel everything was thoroughly addressed in our 1st response letter
This patient is approaching the definition of "harassment", Not only was she explained our complete procedures PRIOR to entering our office, but she also wss given an INFORMED CONSENT (as required by law), which she read and signed and dated
The following is the exact language presented to Ms*** PRIOR to treating her 10/pain scale complaints in our office on June 1, (Noon on a Sunday)
"I hereby request and consent to the performance of Chiropractic consultation, history & examination by the doctorI understand that treatment can consist of spinal and/or joint adjustments and other chiropractic procedures, including various modes of physical therapy and often x-rays, ordered by the doctor
I am willing to discuss with the doctor and/or with other office personnel the nature and purpose of chirpractic adjustments and other procedures prior to undertaking any treatment by theofficeI understand that results, as in any medical procedure, are not guaranteed."
In addition, Ms*** was then, after verbal information on the phone, and this written Consent form, explained when therapy was applied, its purpose
Never during the course of her treatment did she in any way shape or form deny or question anything performed, and after receiving relief from treatment, thanked *** several times for helping her after paying her bill and departing our office!
Attached is the response to erroneous claim regarding an INSURANCE PROCEDURE CODE "-25" on her receipt, she somehow claims as a "$" refernece?
Seeing that she is reacting outside a response, we "in good Faith" refunded a $payment, even though there is NO reason to
We will not respond any further in this matter, and are surprised that an agancy such as Revdex.com, wouldn't see the bizarre and unusual response Ms*** has shown in light of all and any Medical office type settings?

Dear Revdex.com:
 
It is unfortunate and sad that after 32 years of a spotless record in practicing Chiropractic, that this is the 1st time I have ever had to respond to such a disrespectful accusation by a patient, whom I proficiency treated in her best interests! The complaint, which is solely based on “cost”, holds no merit of Judgment. I also understand that a patient has no comprehension as to the legal and moral responsibility of any Doctor, when de[redacted]ng with someone in severe pain! (As she told [redacted] on the initial telephone call, that she was “incapacitated”, and wrote 10/10 pain scale on her history form)
 
The telephone call I received the day after her only treatment in my office was threatening and condescending. The complaint is so bizarre, and scattered in format, that I can only respond to each statement in the patient’s written complaint to Revdex.com, following her outline. My response will follow each sentence in parenthesis, and bold type.
 
The following is the patient complaint to Revdex.com.
 
Customer’s Statement of the Problem:Date of visit was Sunday, June 1 2014. The preceding phone conversations were on Monday, June 2When I initially asked for a cost estimate over the phone before the appointment, the woman was very vague and only said that the exam would cost me anywhere from $160-$200 and that there was no way she could give me an estimate until I was seen. ([redacted] has been working for my offices for over 20 years, and has skilled certification and training as a Chiropractic Assistant, and X-Ray Tech., she has had extensive training in handling telephone appointments, and follows a specific script that covers all possible questions a patient may have regarding fees. As with ALL patients, [redacted] explained that with an “emergency” type appointment, (the patient stated she was “incapacitated”, and wrote on her History form, her pain scale was 10/10), it is impossible to know the exact type examination that would be required until the Doctor sees her, however the range is from $100-$250, depending on severity. This also proves that Ms. [redacted] simply did not focus on what was being said, since, we have no fee of $160, and that is NOT the fee range of our examinations that [redacted] told her. [redacted] wrote in her notes, everything she explained to the patient, who sounded, according to Laura, anxious, and in distress. [redacted] also told me that Ms. [redacted] pleaded for Dr. [redacted] to see her. She also gave a comparison to Ms. [redacted], of entering a Hospital Emergency Room, which would be virtually impossible for the staff to know what Diagnostics and Treatment would be performed upon presentation. The actual Examination fee was $175, so according to Ms. [redacted]s own statement, it was within her thinking.
 
I was never told that the adjustment (treatment) would cost more I was never told that the ice, laser, or e stem treatment would cost additional.I was never asked if I wanted the laser, e stem or ice treatment. (This is plain and simple a false statement from Ms. [redacted]. (Who signed and read an Informed Consent statement, as required by Colorado State Law, authorizing Dr. [redacted] to perform any and all indicated treatment.) I explained exactly what therapies were applied and their purpose to the patient in the office. [redacted] explained that our
Adjustment fee is $60, and that the Dr. will use whatever Physical Therapies he determined helpful to resolve the problem best he can following his examination, she recorded in her notes saying, “he will do all he can to get you wellas soon as possible”.
 
Dr. [redacted] spent at least 5 minutes discussing the school of thought behind Chiropractic medicine and even made a parallel saying that Chiropractors are the Episcopal's of medicine. That you have the Christian church (medicine) and then you have all these Episcopals (chiropractors) questioning everything. It struck me as very odd and I did not re[redacted]ze that I was going to be charged for all of his talking... (1st, Ms. [redacted] was NOT charged for my 5 minutes of discussing various Chiropractic “school of thought”, which she sadly did not comprehend. The discussion was during the initial Consultation, and History review, PRIOR to the Physical Examination. I was simply acknowledging her prior CHIROPRACTIC experiences, in that there are various philosophies within Chiropractic that are different, with the analogy of Catholicism (stating itself as only true Christian Church) as Medicine sees itself in Health Care, and Protestant Churches having a lot of various denominations and differences, as a parallel to Chiropractic healthcare. A common analogy made famous by Dr. [redacted], M.D., best selling author. I’ve never had a patient not see the comparison, until Ms. [redacted], and by the way, the term Episcopols, was never mentioned. The brief discussion was simply to lay a foundation of how my Philosophy of Chiropractic care may differ from her past experiences.
 
After having some time to process and review the statement I received I called Dr. [redacted] to discuss my questions and concerns regarding the previously statedHe said that he did not remember me from the day before (Sunday, when I had seen him in his office) as he had seen a lot of people that day. (Ms. [redacted] called me during office hours, both [redacted] and I were with patients, I picked up the phone during a patient care, and did not hear clearly, who she said she was, and the name did not come through the phone clearly, so I responded that I met a lot of people Sunday, referring to social events later that day, since I did not compute she was referring to being a patient on Sunday. I’m not sure of the significance or point of this statement?)
 
He also said that he was in the middle of seeing a patient and would be happy to call me back a little later.When he did call back (Respectfully, I called back as soon as I was finished in the office)
 
and I tried question the charges that were made and to discuss the possibility of coming to an agreement, Dr. [redacted] became very defensive and argumentative saying things like So youre saying that I dont deserve any compensation for driving away from my family thirty minutes each way to see you at noon on a Sunday? To which I responded, That is absolutely NOT what I am saying. (This is a complete false statement, Ms. [redacted] stated very clearly that she would make known to the entire Valley area, that my prices were “outrageous”. (Which is deformation of Character). When asked by Ms. [redacted], why I charged $75 “after hours fee”, (at Noon on a Sunday), I asked her what she thought I should be paid for coming in, along with Laura, my C.A. when no other Doctor in the entire Valley would,…. She answered, “NOTHING”, then
I got defensive,…. Doctors of all professions, have a standard hourly rate for their time and travel, applied to Courtroom depositions, and the like. $75 is extremely LOW statistically, most charging between $150-$300/ hr. (I believe a Plumber or Locksmith would charge more for a Sunday Emergency?) This was NOT a daily appointment during regular daily hours? When my wife called me around 10:30 or so a.m., I was fishing with my son, and Grandson on their last day visiting from Kansas City. Upon Ms. [redacted] entering our office, her 1st statement to [redacted] was, “Thank you for coming in to see me today, I really appreciate you coming in”. Upon leaving the office, after stating, she could breathe easier, and had relief of the intense pain, she again, thanked us several times for seeing her.
 
I am saying that I am very willing to pay you what is fair for the services rendered. (Her statement of “fair” is not based upon any foundation. Her ONLY 2 issues stated by telephone when asked, were a $10 fee for an Ice Pack, and my after hours $75 fee, and that was it! (The State of Colorado allows $12+ for an ice pack under Workers Compensation Laws.)
 
Also he said, do you know what a 990203/13 costs? (What Ms. [redacted] doesn’t understand is the Federal and State MANDATORY EXAMINATION codes required to bill for Examination fees, so she needs to compare apples to apples, when questioning fees.)
 
He said that his prices were the same as any other chiropractors office in the valley and that he charged exactly the same amount as Dr. Dave (Dave Jensen at the Win Institute) I called the Win Institute as well as Sopris Chiropractic in Carbondale to see what they charged.When I called both Win Institute and Sopris Chiropractic I was told they charge $120 for a first time patient exam and treatment.
(It is highly probable she didn’t ask the correct question, she received a [redacted] examination code, it would be highly unlikely, that 2 other offices have the identical fee for such a code, and it would be impossible for them to know what code applies to the patient until they actually examine them, or what treatment code would be used, prior to examining the patient. Examination codes include complexity, and time factors. There is NO specific fee guideline for any D.C. to charge, as it is determined by each individual D.C. Colorado State Practice laws require the D.C. to document their Examination code within established guidelines, fees are NOT recommended or determined by law. There is NOT one Examination code available to the patient. (These codes are explained in the DORA State Statutes of practicing guidelines.) An Adjustment code is [redacted] depending on what areas of the spine and body the Doctor treats, so to have a 1 fee fits all, is impossible to define according to the law. The fees of another office are irrelevant to our office, the same as Valley View Hospital vs. Aspen Hospital fees.
 
Both of those offices denied having any extraneous treatments that would involve additional fees (such as ice, laser or e stem) (Again, this is an incorrect statement by Ms. [redacted], as I have a case in my office, with notes from one of these offices, SHOWING THE USE of Physical therapies for additional fees, and I have seen countless patients from both these offices, and have reviewed their patient records showing
the use of Physical Therapies. The statement of Ms. [redacted] simply does NOT concur with patient records in our possession.
 
I would be happy to pay the going rate which is $120 plus the additional $75 dollars for Sunday/weekend travel as Dr. [redacted] did graciously see me on Sunday. There is no “going rate” in health care. (When I specifically asked her what she thought I should charge for my C.A. and I being away from my family & guests for 2 ½ hours in the middle of a Sunday, during our last telephone conversation, she stated “NOTHING”!???) Then Dr. [redacted] hung up on me! (I live in Marble, Colorado, near 8,000’, there is no cell phone service from the outskirts of Carbondale to Marble, as Ms. [redacted] herself KNOWS, I assumed either we were disconnected, or she hung up on me? Regardless, after she made threats to tell everyone she knows for over 30 years of living in the valley, I had no interest in continuing her hostile conversation filled with threats.)
 
 
I called back a little later and his secretary (Wife?) answered shortly. I explained that I wasnt sure if we had been disconnected but that I would like to speak with Dr. [redacted] to resolve our conversation. She said hold on. And when she came back to the phone, she said (shortly again) hes busy, as she was trying to get me off the phone, I said no problem, would you please have him call me at his earliest convenience at..... 9[redacted]6 and thank you for your time. (I don’t take kindly to a disrespectful patient, who insults the VALUE of my care! I am semi-retired, and my wife who is thoroughly trained works with me, as do many Dentist, and Medical friends I know in semi – retirement, what’s the point of the statement?)
 
(I wrote Ms. [redacted] a letter, telling her, that any future communications need to be in writing, since she chose to threaten my reputation that has and is spotless for over 32 years!!!)
 
*NOTICE* --- not ONE TIME did the actual Diagnosis or Treatment response become an issue either on the phone, or in this complaint, which gives credibility to the FACT that she received excellent Diagnostic Care, along with the experience and knowledge to apply the correct Physical Therapies, and Spinal Adjusting technique, to quickly address her “Incapacitation”….? I DO NOT do short cuts to detail, especially when a patient claims to be 10/10 pain scale (which by definition describes an “unstable” response to pain, and medically defined as an “exaggerated” claim, unless the patient is in labor, or has bones protruding out? I cannot recall ever having a patient in 32 years that wrote 10/10 on their history form, I’ve had many 9/10’s, but never a 10/10) Any patient stating a 10/10 pain scale, has to be dealt with in extreme caution, and detailed examination care, as there are possibilities of Herniated Discs, spinal cord compromise, and other serious and often life threatening risks associated with a 10/10 pain scale.
 
Had she NOT seen me, perhaps her pain and suffering would’ve played out into more Diagnostics, treatments, by more Doctors, or additional Adjustments, due to poor techniques and other care that would’ve cost additional expenses. I did exactly as I promised, and
my experience and expertise proved it, or I think she would’ve been complaining about the “poor response”, which she didn’t????
 
I will NOT compromise my skills, expertise, and knowledge applied in this case, or the JUSTIFIABLE fees which HELPED the patient, who practically begged me to come in on a Sunday and treat her,…… My Curriculum Vitae surpasses any Chiropractor in this area, and I should rightfully be compensated for it, the same as any highly experienced Physician in their respective field. Ms. [redacted] should stay with the Cheapest D.C. she can find, because they know what they’re worth! She should show some respect to the fact that I properly diagnosed and correctly treated her with the most effective and latest Mod[redacted]ties available for her best interest. To be a “10/10” pain scale, and then have extensive relief within minutes to hours is the exception, not the rule for any Doctor, then to attack that caring Doctors’ fees, for such help?
 
Its QU[redacted]TY, not Quantity that separates a Doctors experience, skill and relative value.

I am rejecting this response because:
It appears as though Dr. [redacted] has misinterpreted my initial complaint which was the non-disclosed itemized add-ons to the initial bill. The secondary complaint is that when I called Dr. [redacted] to discuss the bill on the following day, he was unprofessional and defensive towards me, unwilling to clearly and completely explain as to why the bill was $405. 
 
Lastly, it has come to my attention that a mistake was made in the billing process as it states on the receipt I received (there is a copy of the receipt in the attached document). At the top left of the document (Highlighted by Dr. [redacted] office) it is written that a new patient office visit will receive a $25 discount with additional services.
 
I look forward to resolving this issue.
