Review: I received a letter from a collections agency about 2 weeks ago, stating that we owe $3,336.36 for some blood tests performed back in 2014 by HDL, Inc. Prior to this I had never seen a bill or even heard from this company. I had been assured by my doctor that I would never be billed for these tests - they explained that HDL, Inc. bills the insurance company, and if the insurance company does not pay for any of the tests then HDL, Inc. basically eats the cost. I even found a pdf of the Information for Patients brochure put out by HDL, Inc., which reads "For patients with PPO/PS/HMO insurance, HDL, Inc. will accept the amount your insurance company allows for each diagnostic. In other words, your "out-of-pocket" cost is ZERO for Initial and Follow-Up testing. (HDL, Inc. takes all the risk if your insurance company does not pay for the ordered diagnostics.". The letter from the collection agency states that the account was referred for collection on the "unpaid insurance portion of treatment". After some research, I learned that HDL, Inc. is attempting to collect from patients that they believe received money from their insurance companies that should have been forwarded on to HDL, Inc. I contacted my insurance company to see if they had even been billed by HDL, Inc., and learned that they denied payment to HDL, Inc. because they had requested additional documentation from HDL, Inc. and my doctor's office to prove the medical necessity of the tests that were run, and did not receive any response from HDL, Inc. or my doctor even after 2 attempts. HDL, Inc. cannot collect on this so-called "unpaid insurance portion" because my insurance never sent any payments!Desired Settlement: Provide documentation showing that I no longer owe HDL, Inc. any money, and no further attempts to collect from Monterey Collections or any other collection agency. I also want this to have NO impact on my credit report.

Review: I received a notice regarding a bill from from 2013 stating we had been turned in to a collection agency. I have tried to reach the company representative (contact info given to me by the physician who ordered the lab tests) to resolve the matter and he won't return my calls. There was a 30 day timeline attached to the collection agency letter, so time is of the essence with regard to resolution. The representative's name is Jerry C[redacted], his phone number is [redacted]. I would just like the opportunity to speak with him to see what this is about. My records show the bill was taken care of in 2013 by the insurance company.We have never received a bill in the past 3 years, so this was a complete surprise.Desired Settlement: I would like to speak with the company representative to resolve the issue, and to determine why a bill has never been sent if we have owed money for 3 years.

Review: An attorneys office claiming to represent the Health Diagnostic Laboratory, Liquidating Trust, Chapter 11 Bankruptcy Case No [redacted], sent a letter to this office advising and outstanding balance with their office. We telephoned the office as directed. The person who could barely understand or speak English advised that she would not give out any information about the outstanding bill. That we needed to just send a check. They were advised that we would not do this without verification of the patient, the date the service was rendered and proof of the original bill. They advised that they could not give us any information we needed to just pay the bill.

The attorneys office is [redacted], Attn: [redacted], [redacted]. [redacted].Desired Settlement: I just wanted to put this on here for information purposes only. This appears to be a scam.

Review: May 2014 - a doctor new to me ordered lab tests - not informing me of the nature or need. I got a denial of claim from Premera for the bill that was over $3800. I spoke with the Drs office and was told they would remove the claim and take care of the bill. May 2016 - received a collection notice from [redacted] for $3473.47. This company is no longer in business as of May 2015 - how are they able to file for collections against me? The Drs office - which is still in business - will not return my calls to address why this matter was not resolved two years ago. How can I preserved my credit?Desired Settlement: Have this claim against me removed immediately with no marks against my credit.

Review: Health Diagnostic Laboratory was involved in a DOJ filing where they were involved in a kickback to Doctors who ordered a huge list of blood work. I was one of those patients of [redacted] of [redacted] in McKinney, TX. I was told I was not responsible for those test. Now I am being billed by a collection Agency - [redacted] of Jacksonville, FL. They want over $4900.00 dollars. This Doctor was paid by HDL and HDL was sued by the government. My insurance denied paying HDL. I do not feel I owe this as this was an illegal practice and did not request these test nor needed them. At most the company should apply a discount and then I might owe 20% of the final adjusted discounted amount.Desired Settlement: This either needs to be dismissed or amount adjusted to under $200.00.

Review: I have been sent to collections for a total of 1053.47 from an invoice submitted to my insurance in 2013. Service was provided on 3/16/2013. My insurance covered 8.02+17.08= 25.10 this is the agreed about between the parties. I have spoken to HDL over this claim sever times and now they have sent me to collection trying to get more money that is not due to them based on the agreements made between HDL and my insurance carrier. I do not owe HDL any additional funds.Desired Settlement: I would like HDL to provide me a letter stating that my account is paid in full.

Review: I'm being billed by a collection agency/attorney [redacted] for a bill I never received for a blood test I believe was taken in either 2013 or 2014 - I'm not sure since I've never been billed for it. However, my doctor uses this medical company Health Diagnostic Laboratory Inc and in previous years my insurance pays a certain amount and the rest is written off as an agreement with the insurance company and HDL. I think this notice by collections is incorrect and that the services have been paid in the amount of $1,420.56 and that I don't belong on some collection roster.Desired Settlement: I'd like my name removed from the collection agency and a letter sent to me stating I do not owe $1,420.56 and a correction sent for my credit report.

Review: My doctor ordered a blood panel for my annual physical in October 2014 and my insurance [redacted] denied the claim as medically unnecessary. I did receive notification from [redacted] advising I had 180 days to appeal but having no knowledge I simply ignored thinking the doctor would be notified and resubmit the filing. I never heard anymore until receiving a letter from [redacted] of Jacksonville FL advising the matter had been turned over to them for collection in the amount of $2909.09.

I do not feel I owe as it should have been covered under my insurance and I had the same procedure in my annual physical the year before and that was covered. I further understand from the doctor's office that the same was encountered by other patients and that [redacted] had severed business with Health Diagnostic and that the company may have sought bankruptcy protection.

[redacted] is threatening to damage my credit if not resolved which is all I seek to avoid.Desired Settlement: Avoid negative report to my credit

Review: I have received letter from [redacted] stating that I owe Health Diagnostics Laboratory, Inc. $2,807.68 for lab work done on 01-22-2015. My date of birth is [redacted] has always paid my bills without fail .Upon phoning [redacted], I was told that they have no record of such a billing. I, too, did not receive a bill at any time. The work was done at [redacted] in Port Charlotte, Fl. I do not know if this is a scam, but obviously their billing dept., is at fault for not billing.Desired Settlement: I do not want to receive any contact from Health diagnostics or the billing agency again. I will contact Social Services if I have any contact from a billing agency again
