Please allow me to apologize for the late response. We have just received this complaint via regular mail.
I have looked into this member’s complaint. [redacted]...

[redacted]. She states “I wrote a letter to cancel the insurance” We have never received any cancellation letter from this member.
The member signed up for a Membership on 10/**/2011 and the membership became effective on 11/**/2011. We mailed her a fulfillment kit on 10/**/2011, 11/**/2013 and again on 11/**/2013. I have attached the brochure for you. You will see in our terms and conditions that we have a simple cancellation process. Members can cancel the plan either in writing, email or by doing a cancellation recording. Upon cancellation member will receive a cancellation reference number and all payments will stop going forward. It also states in these terms to please call ###-###-#### to confirm that your request for cancellation was received. Member is stating that she sent this cancellation letter when she received employment in August of 2012. Can the member please supply us with a copy of the cancellation letter and proof that she mailed it so that we may review it? Member is stating that she had one claim pending which was not reimbursed. The claims department informed us that the member did put in a claim for a date off service of 07/**/2012. The member was discounted for that claim by 25%. Please note that this was for an ER visit and member would have to file an ER claim form. We are always more than happy to help out members with any claim. I have attached the member’s brochure and certificate as per her request for what is covered.

We understand that the member wants a refund of $2,787.30. That would be a refund for 19 months. What we do not have is a cancellation letter from the member and or a recording from August of 2012. I have attached the cancellation recording that was completed on 03/**/2014 and at that time the member understood that there was no refund due. The member did not object to and or disagree  to or even mention the fact that she canceled the plan prior to 03/**/2014.  Can the member please provide us with the cancellation letter from 08/2012 and proof that it was sent to us. We would like to resolve this matter just as much as [redacted].

Review: I purchased health insurance through Health Options 1 (Patriot Health UCA) in 2011 while I was unemployed. Upon permanent employment in August of 2012, I wrote a letter to cancel the insurance. I had one claim pending which was not reimbursed. I noticed recently that they continued to access my debit account after I canceled and have been taking $146.70 for 19 months under the name of Patriot Health UCA. I called them to find out who they were and what the charge was for and they contend that they only take verbal recorded cancellations over the phone. I promptly cancelled and was provided with a cancellation reference number on 3/**/2014 for accountDesired Settlement: I would like an explanation charges of what they covered while I paid for coverage from 11/2011 until 3/**/2014

An explanation and refund for why my letter of cancellation was not acknowledged in August of 2012.

reimbursement for medical during the time I was insured and a refund for the last 19 months that they charged me after my written cancellation. $2,7887.30
