payer on 1/16/15 for reprocessing without the discount. I provided my direct contact information for provider to reach out to me if there are any further issues with this claim.
Review: Supposedly signed a contract in 2002 for this plan. Sent me a copy of contract there is no signature of ours on it. They say it renews every year unless we cancel it. Havent gotten anything from them till now. Shorted us on a payment on a work compensation claim.Desired Settlement: Just what they owe us on the difference.
