I am rejecting this response because: My health insurance covers one preventative visit every calendar yearThat is the visit I scheduledThe physician saw me, asked about current/ongoing conditions- without notifying me that the visit type would changeI do not feel I am responsible for a visit that my insurance covers 100% when the visit is coded correctlyHad I known engaging in conversation with the doctor would have resulted in a bill to myself, apparently changing the way my visit was coded, I would not have spoken to him at all

Dear Mr. [redacted]:I understand Mr. [redacted] has concerns about pharmacy billing practices for prescriptions received on April 2, 2018, from HealthPartners Maplewood Clinic Pharmacy. Thank you for bringing his concerns to our attention. With complete insurance information, prescriptions can be...

reprocessed at the pharmacy where they were obtained for up to 60 days. After that time, a request for reimbursement must be submitted to the insurance carrier. Please understand that consistent with Minnesota and Federal Law, we cannot provide or disclose private information to you without a signed and dated release form from Mr. [redacted]. We will contact Mr. [redacted] directly to resolve this matter. Again, thank you for bringing this to our attention.Sincerely,Alecia D.Senior Consultant, Member Rights and Benefits
