wasunrepairable** *** said she would attempt to contact her clientOn December 8, 2016** *** spoke with E*** of HealthPro and stated that she was able to speak with M*** andtold her there were three avenues she could pursue1) Put the claim on her homeowners/renter'sinsurance2) Produce legitimate receipts and or pictures, or statement from retailer indicating, condition and the fact item could not be repaired, or statement of estimated replacement costs toHealthPro, or 3) File a police report and have them speak to the HealthPro employee regarding theincident, and then HealthPro could use the report to submit to our insurance company for replacement,or get an idea of replacement value to reimburse her for damaged itemsAfter speaking with M** ***,her case manager, ** *** relayed to E***, with HealthPro, client's words were "l'm done withthis situation", and she chose to dissenroll her homemaker services, provided by HealthPro, via FranklinCounty Senior Options program. After the alleged incident, HeathPro did have the employee, M***, who has worked for HealthProas a State Tested Nursing Assistant and Homemaker for years, complete an incident reportHerreport is (exhibit C), and states that the unattached mirror did topple over, however it did not damageany of M** ***'s personal property, as she indicated.Inclosing, HealthPro Inchas been in business of providing Medical temporary staff to all medicalinstitutions throughout Central Ohio, and beyond, as well as, Home Healthcare Services to individuals intheir private homesOur Company has been in business since HealthPro is governed by bothState and Federal regulatory guidelines and we are Medicare and Medicaid CertifiedWith the hugevolume of clients, we service, we cannot reimburse clients for damages they claim are caused by ouremployees without doing some type of investigationWe also need to assess the value of items whichclients may say have been involved in accidents caused by our employees, while providing servicesInthis case, the client has not cooperated in having a representative of our agency go to her home and talkwith her and see what damages were supposedly caused by our employeeShe has also provided noconcrete evidence of what the value may be for the items she alleged were damagedIn her firstcomplaint, she never indicated that a water fountain was even involvedLater, HealthPro received a faxcopy of a statement sent from herself, to herself, on November 7, regarding a damaged waterfountain, and the incident supposedly occurred on November 8, 2016.HealthPro does not have a problem in reimbursing M** *** for any legitimate damage which may havebeen caused while one of our employees was working in her homeHowever, M** *** was askedseveral times to have one of our staff members come to her home to discuss the incident with her, assess the damaged items, and receive a replacement cost of the items involvedI have also attached awritten statement from the homemaker, M***, as to what she claimed happened while onassignment(exhibit C)Our company cannot reimburse every client who calls stating that somethingwas damaged, based on a phone callWe must investigate the allegations, and determine, if ouremployee was at fault, and if so, what the replacement costs would beWe then would reimburse, orreplace the damaged item(s)In this particular case, the client refuses to have us investigate theallegations made, and based on the dates, and information provided on a Walmart receipt, which showsan even exchange for her humidifier, and a statement from her IPhone to her IPhone which clearly lookslike she did, and dated a day before the alleged incident took place, we cannot move forward with anytype of reimbursementHealthPro is not denying that the incident may have occurred, and items mayhave been accidentally damaged by one of our employees, we are trying to get proof that if ouremployee did damage the items on November 8, 2016, that we get a realistic value of replacement cost.We would like to get this situation resolved, but until the client allows us to investigate the incident andvalue of items needing replaced, we cannot move forwardPlease note that the pictures provided tothe Revdex.com were NOT provided to HealthPro when requested.Regards,Andy R***

The information that was given you were incorrect. The appointment was made for 1:30 PM. At 1:40 PM, I called Health Pro and was told that they were on their way. They did come and took pictures. All of the statements that was in their report was false. We couldn't even put the water fountain pump in the fountain to see if it worked. I was never asked to put water in the fountain. Why should I, when we couldn't even put the water pump in. Yes, I said I was an electrician and put the screws back because they couldn't I asked to help because they were fumbling and couldn't do it.. The lights did come on but there were no sound. They were willing to offer a total of $50. The estimate was by the Policeman ( A Man) not me.I had to go to the Police Station to get the report. I didn't know what he wrote until I read it.The lamb shade was plastic and we found a piece of it behind the night stand. It was stated by them that the grain of the plastic was not the same. Three shades came with the lamp. When I purchased the lamp, I didn't check the grain of the shade. They came as a set.I have tried to find a match for the shade but there's no match for one shade. I did buy 3 shades but they were not plastic and was to heavy.I have purchased a Homedics Soothing rain fountain for $198.00. I even went on the Internet to find a used one. On EBay, the bid started at $50.  I lost for $87. I sent a copy of the the bid.I also sent a copy of the fountain.I am not going to try to purchase 1 mismatch lamp shade to go to my 3 pole lampIt is hard to believe that Health Pro is refusing to reimburse me for damages. I am not accepting their offer of $50. The minimum that I will accept is $100. I have tried to cooperate, This is my final response to their ridiculous offer.

This letter is being sent to update you on HealthPro's attempt to resolve the complaint filed by Ms.[redacted]. On January 23, 2017, a message was left at 12:12 pm with Ms. [redacted], requesting to set upa time where a HealthPro representative could go to her house and assess the damages for items sheclaimed were because of an employee accidentally knocking over. Ms. [redacted] did respond to themessage left for her at 12:30pm. She did agree to let [redacted] the Manager of ourPassport/Options program, through which Ms. [redacted] receives services, to meet at her home on January25, 2017 at 11:30am.On the above date and time, both Ms. [redacted] of HealthPro and [redacted], of HealthPro went to Ms.[redacted]'s home and met with her. Ms. [redacted] showed them what was damaged, and Ms. [redacted] tookpictures, as reference of the alleged items. The plastic lampshade which Ms. [redacted] pointed out as beingbroken, was one of three which were on a pole lamp. Only one shade was broken, and how a plasticshade broke, hitting the floor is questionable. Ms. [redacted] then showed Ms. [redacted] a musical waterfountain. Ms. [redacted] did take the water fountain apart to display the circuit board, which Ms. [redacted]claims was destroyed. She claims that the "motherboard" was destroyed, and the fountain would notwork, at all. Ms. [redacted] asked her if she had a repair person tell her that, and she stated she was anelectrician.Ms. [redacted] did put the fountain back together and noted that the lighting worked, but music did not. Theclient refused her to put water in it, to see if the pump was working correctly. Ms. [redacted] also had in heroriginal report, that her dehumidifier was toppled over by HealthPro's employee and did not work.At the time of the visit, Ms. [redacted] stated, the humidifier was no longer in question. She said she hadreceived an even exchange for it, and never showed Ms. [redacted] or [redacted] the humidifier.Ms. [redacted] did take photos of the lampshade and the water fountain. She, however was not shown thedehumidifier. She told Ms. [redacted], HealthPro would resolve the situation, and left her home.Ms. [redacted] brought the information to me, as well as, the pictures taken. I informed her to contact theclient and tell her she would be reimbursed the value that she stated the items were worth in the policereport she made. On January 26, 2017, at 12pm, Ms. [redacted] spoke with Ms. [redacted] and stated we wouldreplace the broken plastic lampshade, valued at $10.00 per her, in police report, and replace the waterfountain, valued at $40.00 per her, on the report, as well, Ms. [redacted] replied, "she cannot accept thatoffer'. She stated the estimates were not hers, she wanted all 3 plastic lamp shades replaced. She saidshe wanted new ones that would all three be alike. Ms. [redacted] stated that the three shade were alldifferent which were originally on the lamp. Ms. [redacted] replied, "I know that, but I want three thatmatch". Ms. [redacted] informed her that we would only replace the shade that she claims was broke, notall three. Ms. [redacted] also offered to just give her a check for the value of the shade and the waterfountain, based on the estimated value she put in police report.Ms. [redacted] replied I'm not agreeing to that, I just need more time to figure out someone to look at theseitems some more". Ms. [redacted] stated to her, can you have the company you told us you brought thefountain to, look at it again? Ms. [redacted] replied "Oh I don't know or remember his name, he just doessome work for me. I don't remember his company. After the call ended, the Director of Nursing forHealthPro, [redacted] called Ms. [redacted] and told her we would be glad to reimburse her for thevalue of the broken items. Ms. [redacted] said, "I cannot agree to this, I'm just going to need more time".In closing, Healthpro has made a serious attempt to reimburse the client for the items she claims werebroken accidentally while one of our employees was working for her, in her home. She was offered themonetary value for each item, which she stated they were worth in the police report. The client refusedto settle. Healthpro is more than willing to come to terms with the client for items she stated werebroke, however, there are many inconsistencies in her stories of what was damaged, and the value ofthe items. She first stated one plastic lamp shade, one water fountain motherboard and a dehumidifier. She now states she wants all three shades replaced, the motherboard appears not to be the problemwith the fountain, since the lights do come on, and as for the pump in the fountain, Ms. [redacted] refused tolet Ms. [redacted] check it by putting water into it. And lastly, there is no concern about the dehumidifier, ofwhich she did not have out to be assessed for damage, claiming she got an even exchange for it.At this time, there is nothing further Healthpro can do, since the client is unwilling to settle forreimbursement on value she stated the items were worth. She was told that we would be happy tosettle this claim, however she states, "she needs more time". Until client agrees, HealthPro has put thismatter on hold.If you should have any further questions, please contact our office at ###-###-####Thank youRegards,Andy R[redacted]DirectorHealthPro, lnc
