I have sent the information in reference to Mr *** treatment diagnosed and estimated cost It was explained very well to Mr*** and again he said he understood Additional treatment was added to that appointment therefore visit was added to visit which again Mr*** told me he understood and incurred additional monies which he then paid The amount was less than the due to insurance paid more than anticipated which was a savings to him Mr*** is the one that stated he would not be returning not myself and again he was the one with the extremely raised voice
Healthy Smiles of Gilbert stands firm that no monies are to be returned to Mr*** and it is him who owes us an apology due to I already apologized to him in front of several Team Members.

Mr*** met with Ashley G*** to discuss treatment diagnosed and estimated cost for treatment on July 25, Mr *** scheduled his appointment for the crowns needed and at the time of the appointment while he was in the dental chair Mr*** discussed with DrDavid to
add the implant placement that same day which you will see by the letter attached it was visit The attached letter will give you more detail and is the letter that I sent to Mr*** in response to his letter, today I do need for you to know that when Mr*** came into the office to discuss the statement that he received in the mail he was extremely loud I was on a conference call and had to exit the call to see what the yelling was about When I, Louanne Sanders, Practice Administrator went into the Treatment room where Mr*** was discussing the statement with Ashley G*** he was still very loud and was very rude and continued to be that way the whole time he was in the Office of Healthy Smiles of Gilbert I had several patient's whom could hear him at the back of the office and complained about his rudeness to the dental team DrDavid Gibbons also heard him which in turned DrDavid asked me to dismiss him from the practice which I did by certified letter.Mr*** is not owed any monies from Healthy Smiles of Gilbert As you will see from his dental ledger and dental treatment plan he has paid what he owes for the treatment he received It is all about miscommunication and him not understanding the cost of his treatment which is very clear on the dental treatment plan as I am sure you will agreeWe strive to give our patient's great customer service and go out of our way to make sure they understand everything and have seen patient's in the past multiple times to discuss their treatment and their cost We attempted that with Mr*** and it is unfortunate that he still does not understand

I have sent you the signed agreement between myself and Healthy Smiles of Gilbert. If, as stated there was any change to the agreement an amendment should have been written. There was no amendment written because no changes were made. I new charge of $1,648.00 was written on top of the agreement above and beyond the $1,418.00 to off set what was added by the Dentist, as I was explained, and I paid that amount without any amendment. However, when I got home Ashley called me and told me she made mistakes and now I owed $800.00? I disputed this charge and a week later she sent me a bill for $552.00.
I already asked David about the charges and he told me he did not understand the billing and I should get with Ashley and discuss it. I went to the office to tray and work this out and no one could tell me why I had a written agreement that meant nothing in their office. No one could explain why Ashley called me and demanded $800.00 and no one could tell me why I suddenly owed $552.00. So, I did raise my voice when, after I asked to speak to the manager, Ms. Louanne Sanders came around the corner and told me Ashley was right and I was completely wrong and if I did not like I could go someplace else. Then suddenly she calmed down and told me we would get this all worked out and set my next appointment for me and Mrs. [redacted], who was with me.
It was not until I made an appointment with another dentist the next day, to complete the work started by Healthy Smiles, and they called Healthy Smiles did I get a letter from Healthy Smiles terminating their services.  
They owe me an apology and $782.00.
