ORIGINAL COMPLAINT:
The dentist Harjot Brar with Health teeth Family dentistry REFUSES to give me my records, my new dentist faxed over a signed release form, and she refused them tooI need them to further my dental issuesI've offered money, gave the office as much time as they felt they needed, and she still refuses to release a copy of my dental records and procedures
I myself REQUESTED my records originally in January/February 2015. I left the office a message requesting: ANY AND ALL RECORDS AND PROCEDURES, offered to pay, left my telephone number and asked to be called when they had the "RECORDS" ready. I received a phone call days later that they had a manila envelope ready for me to pick up. When I went to pick up the envelope, I opened the envelope in her office and it contained the following:
Copies of all forms I signed, and a photo copy of my x-rays. THAT WAS ALL. I asked the receptionist where the rest of my records were, she replied that what was in the envelope was all the records the dentist was willing to give me, I asked again if I could get copies of all he procedures that had been done, she asked the Doctor, that would not talk to me directly, by the way, I heard the dentists response, and her receptionist repeated, NO she was not going to give me my any more of my records. She did however print out a STATEMENT, showing claims and payments from my insurance company.
So I found a new dentist in September, and explained why my procedures and records were not complete, that's when THEY had me sign another form for release of ALL RECORDS AND PROCEDURES, hoping perhaps the dentist would send ALL information to an accrual dentist, and not me, and they received a faxed copy of my xrays and statement. Still no procedure recordsThank you,

There is no truth to this complaint filed by your client , Due to HIPPPA privacy act , I cannot give you any details except that this patient had acted  very violently and abusive in the office and  charges of Assault  have been filed against her in the  glendale police...

department and we have restraining orders against her . Thank you .

I am responding to you in regards to the letter that I received in the mail today regarding the complaint [redacted], the dispute being that we have not given the information to the patient about what treatment  was completed  on her . We had actually provided to  the patient , the copy of the ledger which gives all the details of the treatment that has already been completed. It gives you the whole picture-line by line what treatment has been completed. So I would strongly recommend to the patient to show that ledger to the dental office where she will be undergoing the treatment. Please let me know if you need any more information .Hopefully this case will be resolved . Thank you.

Dear Madam,I am responding to the letter that I recieved in the mail regarding the complaint filed by one of our patient [redacted] that we had not forwarded her Xray to her new dentist . She had signed the release form but she never gave us any information regarding the address or email...

where the Xrays needs to be sent .These are patients medical records  and just sending it to anybody would be a big HIPPA privacy violation. If she needs us to send the Xrays, she has to give it in writing with her signature so that we can have all the information in her file .  Please let me know if you have any question.Thank you so much.
