At the time of phone intake for services, on 8/1/*** *** called to schedule an appointment for herselfWhen asked who her insurance was, she indicated it was ***, which is different from *** All clients at CCPC-Ohio are educated at the time of phone intake what
insurance companies therapists are contracted with, *** *** was contracted with ***, and the practice was not contracted with ***, so she was scheduled with *** *** in our Centerville practice because that was the location *** *** chose CCPC-Ohio does not have a Columbus office At the time of phone intake, all clients are educated as to how to check their outpatient mental health benefits and that it is important for them to know those benefits They are also educated on this call and via the fee agreement and consent for treatment that they sign, that quote of benefits doesn't ensure coverage and if insurance denies, they will be responsible Benefits were not quoted to her during this call, just that *** *** was covered under ***, the insurance information the client gave us over the phone *** *anders, like all clients, was told to call her insurance company to make sure that she had covered benefits for services and was told about Page of the fee agreement (available on our website) that she was going to download prior to appointment and fill it out This form gives step-by-step questions to ask her insurance company about her benefits by asking her insurance company the questions outlined there, if she had done this, they would have told her that *** *** was not contracted but *** *** did not fill this form out The fee agreement and consent for treatment includes the following wording:You have two methods to choose in paying for your counseling services:Self-payment for servicesUtilizing your health insurance. In the process of taking care of outstanding balances, CCPC-Ohio noted that *** *** had not paid on her outstanding balance and she was invoiced for $on 3/10/ This was her third notification of her outstanding balance.In the process of taking care of outstanding balances, CCPC-Ohio, noted that *** *** had not paid on her outstanding balance and she was invoiced for $on 7/23/ This was her fourth notification of her outstanding balance Based on no response after this 4th notification, and waiting days from the date of last invoice her credit card, that she put on file as documented in her intake paperwork, was charged The credit card guarantee that she signed states the following:As you may know, if you have ever checked into a hotel or rented a car, one of the things you are asked for is a credit card, which is later used to pay your billThis method is now common practice in the provision of most services, particularly where you receive the service and then pay laterCCPC-Ohio has also adopted this process and therefore requires a credit card guarantee in the event that your insurance company fails to pay for services (remembering this is an agreement between you and your insurance company) or you fail to pay your co-payments or other fees that are outlined on this fee agreementWe want you to understand that we will only bill your credit card after calendar days have passed since the date of invoice to youYou will be notified that charges were made to your credit card via an invoiceWhen charging fees for the sessions covered by insurance, we will only charge for the contracted rate the CCPC-Ohio (THERAPIST) has with the insurance, or managed care company, if that is the case, minus any payments they or you have made on your claim.I acknowledge after having read the Fee Agreement and Consent to Treat Form that I am obligated to pay any outstanding fees based on the terms listed in the aforementioned Fee Agreement and Consent to Treat Form, and I hereby authorize CCPC-Ohio to charge my credit/debit card for such feesCCPC-Ohio has teamed up with First Data Secure Transaction Management service, a layered security solution that will eliminate the need for CCPC-Ohio to store payment card numbers at the point of sale, complying with the Payment Card Industry Data Security Standard (PCI DSS).In conclusion, at no time did CCPC-Ohio charge *** *** the $billing fee that could have been applied for each month her balance nor was she reported to a collection agency, both outlined in her fee agreement, and she was offered a payment plan which she did not respond to After numerous notifications by both her insurance company and CCPC-Ohio as well as attempts to resolve her outstanding balance, we defaulted to the agreement *** *** had with this practice Sincerely,*** ***

This happened a year ago, so I do not remember every detail of conversation I had with them. Trying to schedule an appointment was difficult as no one seemed to know what they were doing there. I remember being at work when I got a return phone call from them wondering what I had gotten myself into because they seemed so confused.
